Thanks for putting up your case history. On quick read she looks to be definitely IR, maybe PPID. Diet treats IR, pergolide is for PPID. As it's really late I will comment more later. In the meanwhile, have a read in the files and I would immediately implement the emergency diet that you received when you joined.
