I had a horse with this kind of problem and after having tried everything you described, I finally switched him to Alam by McCauley's Feed (he needed grain to keep his weight up and there was only so much BP he could handle). It took six months or so but it steadily improve and what I noticed is that his skin problems cleared up and his weight is the best it has been.
