One of the values of the case history is to be able to see the *details* asked for on the form , in order to determine just how unremarkable (or not) the history is.
 Or how relevant a detail might be.
The devil is in the details.
It would be irresponsible to try to answer your questions without having the advantage of seeing the details.

I see that Dr. Kellon has  already addressed this,so I don't need to say more.
