Spreading cancer isn't a concern since PPID is benign, not cancerous.  There are plenty of issues to be worked out though.

There aren't enough details given here to really comment on it.  Getting equipment to the area is just part of the problem though.  The surgeon also has to be able to see what needs to be removed.  This is a good discussion from the human end:
