Just put the lab results you received into the case history form as is - using the units that you have. Lab units means the reference range that your lab uses, along with what units they measure in.
The results you have are in standard formats, no problem.
Your ACTH is very elevated so Willie is positive for PPID, or if he is already on pergolide, his dose needs to be increased. He is also positive for IR, even if the samples were not handled properly.
Glucose is low enough that proper handling of the sample is suspect. This would also call into question whether the other results might be higher as well.
