Updated Iron levels and most recent insulin and ACTH values- desperate for advice

All values were updated in case history. Top line results below:
Buddy's insulin levels were high 99.75 uIU/mL (range 10-40) Serum Iron was 225 ug/dL TIBC 535 ug/dL Ferritin 244 ng/mL T4 was 0.630 ug/dL (range 1-3) I am not sure how hypothyroid is diagnosed? Would I add Thyro-L here or metformin or both? Also, how do I get the iron levels down?
