It's not true that metformin does not increase insulin sensitivity and only works by blocking glucose absorption https://www.ncbi.nlm.nih.gov/pmc/articles/PMC8836112/ . Since horses are not meal fed the way people are, it doesn't really matter if you give it before feeding, especially if the diet all day is the same.
