Martha is the expert on i-devices but to get you started now, first, make sure you have the Pages app installed on your phone.  Second, somewhere in the app is an option to switch from read-only to editing and you need to turn that on.  Then you should be ok to go.  I can't imagine filling out the form on a phone though, although many of our members do.  A tablet or computer if you have access to one would be a lot easier for you.

Was that bloodwork done fasting?  If not, what and when did she eat prior to it?
