I agree Metformin is reasonable until you can restart exercise.

The iodine in Thyrol-L is not available to the body. Continue normal iodine supplementation.

No reason not to do IRAP.

Jaini,

We ran a ferritin test last fall (Kansas state) and other iron panel. Water is city water. The hay has super low iron. Unless there is high iron in ODTBC, I don't think iron is contributing. Thank you for raising the possibility!

4. I think you may have it right regarding the right hock. The Sports med vet suggested IRAP into both Hocks at same time as stifle injections. Any reason not to do this besides putting me in the poor house?

In searching the past messages, I didn't find anything recent on adding sea salt versus iodized table salt.  I have been adding table salt for several years for my IR horse, but have a friend who would like to add sea salt, specifically Pure Unrefined Sea Salt from The Holistic Horse which lists the following analysis of their product per serving.

I have just had a similar conversation with a BC pharmacy that is selling me a compounded form of pracend. They also claimed that I under no circumstances should I cut any tablets, saying the same, it will disintegrate and there will be next no active ingredients left the next day.

Thanks Jaini,

The single bag I am getting tomorrow is the right stuff. The big order going to Tractor Supply is the Plus. So I will try getting the order changed at Tractor Supply. A different guy ordered it and didn't know what to get.
I think I made Scarlet IR by feeding alfalfa to get weight on her. But good news, she ate the 1/4 lb Stabul 1 with a big 3 handfuls of beet pulp right away this morning! I will try to continues increasing that.
Yesterday was pretty stressful but feet are now trimmed, some feed is coming. I will start working on Tractor Supply again.
As far as the Omeprazole goes, she actually seems to do better or at the very least just as well on ranitidine. I gave up on Q 8 hr. Three times a day when I am here seems to work. Last is usually 11 pm.
What about the importance of exercising her?

Hi, Robyn - I see nothing in your diet that could be contributing, and the ACTH looks to be under control.  I will flag this for Dr. Kellon. One thought - could the gait imbalance from the right hock be contributing to the stifle injury? This gait imbalance need not be obvious, just enough that extra strain is put on the stifle in some situations.

 Given that Toons will once again be laid up due to inury, so minimal exercise, you probably should be looking at Metformin at 30 mgs/kg twice daily. 

Dang, I am sorry you are going through this. Sometimes things are just way too unfair.

Yes, searching the archives can indeed be challenging! The only issue I am aware of with MSM is that it may interfere with copper uptake, so you have to watch your copper supplementation if using it.   What evidence is supplied that suggests MSM might make IR worse?

I'm sorry.  I was going to delete the question this morning as late last night I found the text to the link in another post.  

There was a blog sent to me stating MSM can make IR symptoms worse.  I was looking for information to dispute this to the client and show the issues (if any) in supplementing with MSM.  I am also taking the AOL class and looking at information on Arthritis treatments.  Sometimes searching thru all the posts for a particular fact or gem of information can be a challenge.

Joan, I split a dose and simply keep the half tablet in the plastic blister with the foil over the top of it and use it the next day. It stays in my tack room which is heated in the winter and I run a humidifier in it in the summer. 

If Prascend was that fragile, it would be dangerous for anyone to handle as it's medication would immediately be dispersed onto the skin, possibly inhaled, etc and would have never been approved. Along with all the compounded pergolide.

Eleanor, you were quite right about sugar in the phloem. I chiselled bark off a tree down to the wood one afternoon, and the next morning there were a few drops of sap oozing from the interface of wood and bark at the cut edge that were very sweet to taste. What amazes me is that donkeys, and horses, are prepared to eat their way through up to 1cm of coarse bark to get to a few drops of sugar.

 

Re normal ACTH values in donkeys, I have been informed that the Donkey Sanctuary has revised its normal values upwards since 2011, but their results await publication.

Please, please advise on getting IR controlled - Toons RH Meniscus Re-Injured

 

Toons had an ultrasound of his stifle today. We learned that the meniscus was re-injured. Same area that was injured in 2014 that appeared to be completely healed.

The Discharge notes from today are: Right stifle : hypoechoic line on the edge of the medial meniscus - more hypoechoic and distached than on last recheck in 2014. Mild med FTJ and FPJ effusion.

OK, so the issue of favoring the right hind with some left hind stiffness is somewhat solved, but then . . . the IR is still uncontrolled. 


I am thinking that his uncompensated IR could be a contributing cause of his injuries (annular ligament desmitis (RF, LH - 2016), Meniscus tear approximately three weeks ago (with no apparent or known causal event). Could this be the case?


I am very concerned - if we are limited to hand-walking 20 minutes twice a day for two weeks (adding 5 minutes every two weeks), with a recheck ultrasound in 60 days, Toons will not be getting the exercise he needs to control the IR. If I reduce his hay intake any further, he will be eating dirt. We have new orchard grass that was tested and re-tested with the following results. I am soaking 8 lbs of his hay daily. He is getting 16 lbs total every 24 hours plus his ODTBC (cup) to take his minerals and supplements (now including Thyro-L).

1. Should I add Iodine to his mineral supplement while he is taking Thyro-L?

2. Can you recommend any product to try while Toons is laid up that MIGHT make a difference in the high Insulin, Glucose, and Leptin?

Our current situation is that like I assume many folks on the east coast, I have been stretching my Stabul 1 waiting for the order to arrive at my Tractor Supply. I got one last bag on April 27 and Scarlet is now down to 1/4 lb twice a day with her minerals. I found out that there was an issue at the New York mill that makes it for the east coast distribution. I now have a bag coming Fed Ex from Illinois which should arrive Friday before I totally run out. The only other feed Scarlet is eating is at most 7 lb of hay and 2 lb of TB cubes a day. That is all the hay she will eat and all the cubes she will eat. Doesn't like soaked or dry. Used to eat 2lb soaked with peppermint and Stabul 1 mixed in. Now I am having the most luck with a little water with peppermint put over the cubes like salad dressing. I can sneak 2 handfuls of lightly soaked beet pulp in with the Stabul 1 and minerals. She doesn't like beet pulp either.

I just found out that  the order that will arrive at my Tractor Supply in 1 -2 weeks (hopefully) is the Plus. Scarlet is uncompensated IR.

When I evaluated this. It showed that she was not quite compensated , which was also the case in June 2016. Sorry I did not enter this in case history. The last time I tried to do that I spent much time and only achieved much frustration.

I would like to know if her IR will get worse if I feed her the Plus version of Stabul 1.

She will be 35 in June. I am not exercising her though I guess that is what I should be doing to help the IR. All I need besides feed is more hours in the day. Oh She needs to stay on Ranitidine 3x a day to keep her eating. And my hoof trimmer is finally coming tomorrow after being unavailable since Easter. I am quite stressed. 

Sorry for venting. I am so thankful ECIR group is here or Scarlet would not be.
