Here is an old  post from Jaini,discussing exactly this:

"Hi, all - I know this is an area of some confusion.  Prascend tablets contain 1 mg of pergolide as pergolide mesylate. 

When switching to or from Prascend, make sure to get the vet to write the script as "x mg of pergolide as pergolide mesylate" ,  rather than "x mg of pergolide"  That way, each capsule will have the same amount of pergolide as the equivalent number of Prascend tablets. So, a prescription for 2 mg of pergolide as pergolide mesylate will contain the same amount of pergolide (and pergolide mesylate) as 2 Prascend tablets.

If you are already on compounded pergolide, and are increasing the dose, there is no need to fuss about whether the pharmacy has given you x mg of pergolide, or x mg of pergolide mesylate, because you are titrating the dose up until you get control of the ACTH. It doesn't really matter what form it is in.

Does that make sense?.

Cyproheptadine as an addition to pergolide is not effective. There is no support for it's use as effective in the veterinary literature, except from the maker of Prascend. They needed to give folks a place to go beyond the expense of their drug. Most of us switched to compound instead when this happened, because the cypro does not work.  I am not aware, could not find info relating to cypro and sweating.

-high insulin
-colicky episodes
-hamstring issues
-classic signs of PPID (loss of topline, long coat, etc)

Based on what I've read in this group, these signs all suggest to me that an increase in pergolide is warranted.  You might not be able to use ACTH as a marker for how well Logan's PPID is controlled (ie, if some or all of that ACTH value is from an inactive form of the hormone), and instead you need to look for improvement in the above signs to know whether his dosage is correct.  If he has high insulin (yes he does; share the above explanations for the Cornell insulin range with your vet), hamstring issues, and colicking problems these are things your vet should take seriously.  A trial increase in pergolide needs to be considered.  That's assuming that what you are already giving him is being stored properly and that he is getting the full dose...review his medication storage and dosing with the barn staff.

If your vet is not supportive, you might want to share with him/her the ECIR database on high pergolide dosages.  See this recent topic for more info:

I tried your instructions for updating CH on iPhone several times but what it said to do did not work. It only gives “copy or select all” choices. No choice to drag anything anywhere.

Having difficulty understanding exactly where you are running into a problem however assuming you are trying to move your CH, I suggest you select and copy, if that is your choice. 

Most public libraries have computers. Recommend that as an alternative.

Hi, I just recently switched from Prescend to Compounded Pergolide from Wedgewood Pharmacy. I haven’t started it yet. RC is on 2 mg of Prescend daily. I assumed he would get the same dose of Pergolide. Am I wrong?
On a different note, RC has started sweating under his sheet this year. We have been taking them off at night and putting it on during the day as needed. He has a full body clip, minus the legs and neck, which doesn’t seem to be helping. My vet suggested adding Cyproheptadine, which he has been on for 2 weeks with no change. She suggested repeating his bloodwork in March. Is there anything else I can do?? Any suggestions on blankets that whisk moisture away? I don’t want him to get chilled. Thank you all!!

Good explanation, Amy.  The insulin of 80 (give or take) is an estimate for triggering an acute episode but there is smoldering damage going on with high insulins even if the horse does not show an acute episode.

The 4 hour wait after first  meal of the day is not needed if the horse had access to hay or pasture overnight.

I'm interested that people are reporting their horses' rejection of J-Herb.
The several in my herd  that I used to give it to all ate it as a paste, either out of my hand,or from a small bowl. I'm feeling fortunate.

No specific brand. I would check every 2 weeks or so, or any time you see suspicious signs like frequent urination of small amounts, stretching out to urinate but not doing it, fever, dropping the penis, strong odor to the urine.

Well, maybe I'll contact mybesthorse.com. The smell is smokey, but it reminds me a bit of an old ash tray. And when it is moist, it reminds me of burning rubber. But that might just be me. I'll just have to leave the Jiaogulan out for a couple of feedings to see whether my IR/PPID mare will eat her feed more willingly than what she is doing right now. Thanks for all your comments!

Thanks Deb! And glad to know Scotty continues to improve! In February I hope to get rads of all four feet to get a closer look at where we are in our progress. When my farrier comes next week I'm going to ask him to show me how to rasp and to let me try it just so I can get the feel of what it's like to actually do it. That should be interesting.

I started Scotty with a liquid compounded Pergolide, and after nearly a year of not seeing a lot, discovered that was probably a mistake. There are a lot of files here, including some of mine, that state how the liquid compounded versions do not hold their potency. Who would have known? I have since informed my vet. We now use Pet Health Pharmacy and he is getting 4 mg pergolide which actually equals about 3 mg Prascend, and he is doing well. I get the capsules, and after trial and error making sure they are consumed, I just stick one way back in the corner of his cheek, hold my hands over his face/teeth, and wait for it to go down. Otherwise, he got so he wouldn't even take a treat from me because I had meds mixed in.

Since on/off using it, I have only ordered from My Best Horse. It is very green and lightweight...like a whisper of wind. It has a definite odor and I tasted it and sneezed...kind of like hay powder. Scotty doesn't seem to be fond of it, but I give him some (not before feeding) just in case it helps. His gums seem pink enough to me, but gotta stay on top of these old horses :)
