Hi Nancy,

If in doubt, I would contact Cornell but according to their website both glucose and insulin should be serum (https://www.vet.cornell.edu/animal-health-diagnostic-center/testing/protocols/endocrinology).  Looking at your CH Akira is still overweight and eating an unknown amount of hay and grass each day so it's not unexpected that her glucose and insulin levels are still elevated.  If she has gained a significant amount of weight since your last update that could have a bearing on her current bloodwork but the main takeaway I get from those results is that she needs tighter diet control.

Dr. Kellon may have more to add once she's had a look as well.

What Helen says is true in my experience. We started my mini mule with traumatic arthritis (complications from a broken femur as a long yearling) with Adequan, and it didn't seem to do anything for her. Pentosan does, and Pentosan Gold works even better, but I'm having trouble sourcing it. Vet's currently checking if Wedgewood can compound it, but they only offer regular Pentosan (from another company?) online.

It has a mild analgesic effect, but it is mainly staving off the inevitable with her. I honestly thought I was going to have to euthanize her before she was 5, but she turned 5 a couple weeks ago. She looks terrible to watch her move, but she insists her quality of life is still good - as long as she's happy, I'm happy.

Arizona copper complete is in the custom supplements section of Horsetech. It is somewhat similar to CA Trace plus. You could test one batch of hay and have someone balance it just to get an idea of what you’re balancing to. Mine have always been somewhat similar mineral composition. It would be better than nothing. It’s not really any more expensive to get it custom made, in fact it saved me money over using stuff I didn’t need.

I use Horsetech and they can make anything you want. I have had flax base but it gets so hot here in AZ that the flax tends to get rancid. I prefer to get it without flax, they add just a small amount of brewers dried grains as a base to make the serving size easier to measure, or you can ask for no base at all, I’ve done that before. I prefer to buy stabilized ground flax separate and add it. I sprinkle it on my horses wet hay every morning as she will not eat wet feed besides hay either.

I’ve had the same thing, Lorna.  A big weepy welt but no sign of a tick, until you dig deeper.  Sometimes I think they get rubbed off before I notice them.

Dr Kellon, going back to a question to latest rads. Did you see the same possible evidence of retained infection in the March rads? The antibiotic profusion was done a while back in 2019 I think August.  We just did the tildren profusion in may..

Thanks all, I use BOT and Benefab products and I just got the magnetic bell boots from Veredus Magnetik line.I love all these products too.

The benefab smart scrim and smart neck cover are amazing the horses go into zen land when wearing them. 

I use the BOT as a sheet and neck cover during spring months. And i switch between benefab versi wraps and BOT standing wraps for Vinnie's lymphedema on his right hind.

Hi Lindsay,

The reason I mentioned ticks is because years ago my immediate neighbour told me her horses had no ticks. My horses had tons of them,so this was a real conundrum.
She said her ponies did have some kind of something, though.A skin
thing,swelling,and sometimes scabbing.
Bingo.
That's what the tiny ones do.They are often very difficult to see,very. But a sensitive finger will pick them up.
Sure enough,upon further examination,this turned out the be the skin  thingie.
You probably know all this,but just in case.

New lab results for Akira

 

Hi all ...I am updating my case history with the new lab results but I have a question. When my vet drew them he wasn't sure he sent them off properly spun or processed. I got my new lab results and they are pretty in line with last years except I noticed this years say Serum Insulin baseline and Serum Leptin baseline where as last year they were Plasma. Does this mean I need to redraw the leptin and insulin or are we good for another year. Her labs until I update her case history are:

Serum glucose 92 (reference range 71-122)
Serum Insulin 20.85 ref 10-40
Leptin 8.25 ref 

I am confused because last year her G/I ratio was 4.8 Compensated. This year it is 4.4 Uncompensated

RISQ 0.22 this year  Poorly compensated and 0.23 last year compensated
MIRG 8.8 High Insulin responder and last year 8.2 high insulin responder. I am confused if the numbers are so close why she would have gone from compensated to uncompensated and poorly compensated?

Also the glucose is the first time it has been sent out...the other vet used to test on site so this result came back with a bunch of other things like Lipemia, hemolysis, Icterus, Cobas Interface. I have no idea the relevance of all that or is it just the standard when you send a glucose off? I apologize but it is taking an extra long time to update my case history because that was on my old Windows 7 laptop and moving it to the new Windows 10 and trying to convert it has taken me sometime. I should have it updated soon but was more worried about all the past labs being plasma except the T3, T4 and Free T4 I had in the past vs this year being plasma and a new vet unsure of how to send them off.

Hi Lorna, 

We have ticks.  We don’t have Lyme, but other tick-borne illnesses can be an issue.  (I hate the nasty little bloodsuckers!) But ticks are not Audi’s issue.  We don’t have a ton of ticks on the property, as our poultry eat them.  We *occasionally* find them on the animals, but I haven’t seen any on the horses, and I’m more hands-on with the horses than with most of the other animals.

I don't know if other use them, but Sunday finds relief from Back on Track products - he has a sheet & hock boots which I often use for several hours before riding. When he was boarded & stalled I also used standing wraps with great success. They have shipping boots I believe which should cover ringbone area.

Thanks again for all the info!  I so appreciate everyone taking the time to respond!  I need to run in a moment, but wanted to comment quickly on his eating.

He eats well!  Though I don’t have hay weights at the moment, I do have the opportunity to observe him while he’s having a “meal” and is separated from Mogley.  He always finishes his meal; I don’t think he’s ever left more than a mouthful in the bottom of his bucket.  His current meals consist of: 1.5 lb r/s/r beet pulp, 0.5 lb timothy cubes, 0.5 lb ground flax, and his minerals & supplements.  He gets that every a.m. and p.m. and consumes the entire amount.  He always has some hay in his meal corral also.  That way, if I feed him and then move on to something else, he can munch on some hay while he waits for me to return.  More often than not, a big pile of hay is gone when I return if I’ve been gone for any length of time.  Now, whether he digests his food well could maybe be a separate issue?  Could he be suffering from some sort of malabsorption issue?  Ulcers?

Perhaps it was a salivary gland that was swollen, although I feel like the vet mentioned a lymph node.  I remember that because I was surprised that he didn’t call it a salivary gland, which is what I assumed it was.  Regardless, the large swellings are a transient issue.  The smaller “hives” are an ongoing issue and appear all over his body. He always seems to have a few of those.

Unfortunately, the vet can’t see him right now.  The earliest they can either come here or see him in-office is June 10.  Because I didn’t want to wait that long for labs, I worked with them to draw the blood for ACTH myself, then I scheduled his imaging for the 10th.  I will definitely discuss next steps when he’s in on Wednesday, and I have my fingers crossed that his labs will be available then, too!

I was holding off on insulin until his laminitis resolved.  But I think you are saying to just go ahead and draw that ASAP.  Am I correct?
