The Invokana can't compensate for PPID out of control. He needs more pergolide. How much water does he drink? Are you doing the urine  dipstick checks? What is his specific gravity? Are you wetting hay before putting the salt on it?

Stop the Carb Safe for now. I'd start Laminox for circulation support https://uckele.com/laminox-3lbs.html (coupon code ECIR).

If you haven't followed up on Lyme testing I would do that. His February results were consistent with early infection.

Steglatro trial

I just heard that a veterinarian at Tufts is starting an equine clinical trial with Steglatro, which my vet says is in the same class of drugs as Invokana. I guess the goal would be an equine specific medication in the future. Has anyone used this? I guess I should have done a search in the group, I'll check now!

Dear everyone anyone ,over the past 6weeks or so gavi has been having puffy eye sockets and tender feet.He is currently on 2mg pracend ,we recently switched to compounded version,and 300mg ivonkana.He was doing very well on 1mg pracend and 150 mg on ivonkana before fall rise .we started inceasing meds a bit behind schedule ,my fault I forgot when we needed to to stay ahead.All hay is the same and tested frequently as  we move through this field of hay we are comitted To. Its been great and safe .We are using as fed results hoping that is till acurate.I decided to have him tested last week ,kidney function  which is showing a high normal result. Also  acth ,insulin and then a stim because gavi notoriously tests in Normal range without a stim .He is 200 for insulin and 300 acth .I cant upload history without many
-- struggles so looking for immediate help please.Farrier coming every five weeks as well. He has soft rides and pea stone and always a bed if shavings to fall into .he has leg warmers and wool socks.He gets a little bit of etec carb safe ,vt blend, gut health,phylum powder, chia seed ,he hates flax, salt over hay ,vit E ,he tries to play in the field with his friends everyday as well .metformin is nearly impossible to get in he hates it!!!

Ok i have been thinking about my plan.....

1. Stopped banamine, he was getting 250# dose 1 time per day for a long time at least a year or more.  As Dr Kellon quoted Einstein in a previous post, "doing the same thing over and over again and expecting a different result"... (is definitely not working for me) since he was getting a low dose i will monitor him to see if I need to wean off more slowly.

2. Tried pq, and Vinnie said no way.. so I am going to try the hemp joint from uckele as he will eat that but double the dose 2x per day for the DC. I have ordered cocosoya granules to see if that will be a game changer for PQ.
3. Added mov ease back and he ate that in his mash.
4. Adding pea 2500 mg 2x per day (starting tonight)
5. Increasing jherb by 1/4 tsp per feeding to pink gums. Since he is eating Glycocemic eq and laminox in his mashes I don't want to mess with  good thing so I am leaving those alone.
6. Starting a course of ulcerguard tonight 2 hrs before any meds/herbs as he was very tummy reactive during our last acupuncture appt. On Saturday and recommended I treat for ulcers.
7. Try to figure out a better pad solution for the left front. I have purchased about 8 different boots with bunch of different pads that I can try different cut outs... i have also read that yoga mats worked for some so that is in my list to try(yah I am crazy...so..I may have some boots for sale or to loan out later :) )
8. Red light therapy daily 
9.  Centurion mini pulse on 2hz daily starting in a week when I get a better idea of his pain tolerance off nsaids.
10. He is getting 10ml  apf syringed in the am still and vit e in the pm, I dont want to syringe too much more...(he may boycott my presence:) I could add something to the vit e syringe if needed but I would prefer not it at all possible.

Since I am trying to give jherb in food as well as pea and mov ease should I increase the dosage or should i really be syringing these?

Still giving meds (metformin, prascend, thyrol)... 

Last but not least even though his insulin st time of testing has never been higher than 88 ans was last 42 should inbe considering invokana because of how high risk he seems to be?

Dr Kellon and all, please let me know your thoughts on my plan and if you think anything sounds incorrect?

Hi Alicia,
I thought it would be too expensive for our budget, but I ended up being pleasantly surprised.

The cost is going to depend on your dosage. We got our first month of meds from Costco. It was $630 For 30  300 mg pills for Finn. His dose is 150 mg so the 30 pills lasted 60 days. When Elliott started we also got the first 30 pills from Costco. His dose is 100mg and we got 30 100 mg pills. Also $630 but only for 30 days. Unfortunately I couldn’t accurately split the 300 mg tabs into thirds for Elliott, that would have saved quite a bit of money  

My vet ordered the 300 mg pills for Finn and the 100 mg pills for Elliott from a pharmacy in Miami. I was shocked that it was so much less expensive. Finn’s meds are around $70 a month (because 30 pills are divided for 60 days) and Elliott’s are $118 per month.  They are both ponies around 12-13 hands.    You can probably get it cheaper if you get it directly from the pharmacy in Miami  I’m quite certain I pay a fee for my vet ordering it but they said that was the only way they would prescribe it. 

The app GoodRx has a coupon which saved us about $30 when we got it at Costco. 

It has saved my boys’ lives.

I was having great difficulty finding a pad my horse was comfortable in.  I found using a thick sponge cut to the shape of the hoof attached with elastic on did the trick.

I'm out of the loop now, fortunately, so wouldn't want to add my 2 cents,wrt any pharmacies that have stepped up in the interim.

The owner of Pharmasave (still?), Bob Mehr, was originally at Island, where I met him on my original search,a couple of decades ago. He was great and helped my horses a lot. After some years he moved to Victoria, but I understood he was still compounding pergolide.
I continued with Island,where Kathryn is the one I have dealt with ever since. Until 2 years ago,when my 47-year-old, PPID pony mule took flight.

Have others found a reliable source,which is doable wrt cost?

Alicia - when I first started on it I purchased at local pharmacy - $ 400 for 30 days - then sent an order to pricepropharmacy.com without looking I believe it's $ 270 for 90 days so much more affordable and ships out of the UK.

I used to get mine from Valley Vet and never had a problem getting my vet to send in a script but when his practice started contracting with covetrus, I switched over to them because they periodically give a $25 or $40 discount code on your order which makes it cheaper than Valley Vet. I think the compounded pergolide may be better for my friend in the long run, but right now I just want to try and find her the best deal that she can access. There is an online pharmacy in BC that has the 60 count box for $125 but that’s the only place I was able to find. Also in the US, if there is a legitimate veterinarian-client relationship and the animal requires a medication, the veterinarian has to provide a prescription if requested by the client and cannot force the client to buy the medication from them or their contracted pharmacy. That’s not saying some vets don’t put pressure on people to buy from them.
