-- Commenting on invokana not being a magic bullet like Dr. Kellon mentioned, regretfully I have to agree. Jesse’s AC TH went high and that was on me because I switched up some medicines but I thought incorrectly that the pressure of the invokana will keep the insulin down to normal. And it has not. So I am trying to work this out but I just wanted to mention that invokana is not a magic bullet that I hoped it would be but certainly is almost  half a magic bullet?

Hi LJ.  Did the vet/farrier test feet with hoof testers? And he was sensitive in all areas of the hoof? And did the farrier explain how the shoes would help and why? I like your idea of the boots for comfort while walking...and returning to a five week trim...it would be good to see a few more photos too....

Please start a *new thread* with a *new subject* so that Dr Kellon can efficiently sort out your request from all the other stuff in this thread. In your case, since your signature has no link at any case history, we have no idea which equine you are asking about and what the test results are without reading many posts. You really need  a CH.

I’m very interested in learning more/trying Invokana for my very IR/Cushings pony who has stopped responding to Metformin ...we tried increasing the dose and he stopped eating so I’m looking for a better alternative. My vet has never heard of Invokana ...where can he get it from and find out more info on it /dose etc ??

I would also like to jump in on this topic.Gavi started ivonkana last year .He is a severe case IR /cushings .We started ivonkana last fall and all year his life has been changed a 200 level insulin dropped to 34 in 3 weeks .The comfort came within a week .We are having issues now because his acth is high and that drives the insulin up for Gavi everytime .We just did a urine test and glucose is still spilling out of his urine from the ivonkana ,so i know its doing its job.We are trying to get Acth down that will then  also bring insulin down for my horse.Ivonkana has done wonders for my beloved horse.Before my shipment came in I purchased from costco at 22 $a pill .At 1 point another member also offered me a borrow pack and then I just paid them back as they always order ahead. I would offer as well but ,im running low and new order can take up to 6 wks to 2mths How kind everyone is on here to offer to help.Have faith its gonna work its magic,and can be life changing so hang in there ,fondly, Tara

Hi Christina,

This is why a case history is so important.  Are you administering the Prascend 2x a day as well as the Thyro-L?  Why is she on the Thryo-L?  Are you using or have you used APF?  How has she broken her teeth? What is her diet?

Really sorry to read this and I know how terrible it is to see them like this. My boy cannot have any alfafa at all in anything, even in very small amount. I was not suspecting this and he suffered for months. Juste in case it can help.

I had the vet out to look at jesses feet and the farrier was here as well for a 6 week trim .Jesse is reactive in both front feet but not active laminitis. The thought is that all the extra work is getting him wearing down his feet. Farrier said his right front that there’s never been anything to trim  because he’s always wearing it down from walking around, etc ie increased movement at new barn So I’m going to use the clouds while he’s on  the exerciser , at the walk only  , after a day or 2, and try and get some hoof  to regrow before i consider a trot regimen to reduce the insulin . I’m going to stop the Prevacox  gradually, and restart jiagoulan because I think jesse needs the foot growth. I did send some pictures to his case history file.   The farrier thought he could use shoes immediately. vet thought he wouldnt tolerate the nails.   farrier didnt agree.  i took no video of Jesse trotting very nicely   , ( i was talking with farrier when vet trotted him) I saw Jesse Canter around his pasture when a girl came on a bicycle,, he cantered very nicely. And his trotting  with the vet looked fine as well. Additionally, he is walking much better after his trim  at six weeks. At four weeks I will evaluate if he can go to six weeks or we should move back to five week trim.  I’ll probably require five weeks regardless.

In fact, one of his hoof is really no good and rotated (for real) but the other is not just long toe... there is radiograph, but as I said, many people don't want to go here because it's in english (that's so sad)... but I am happy to know that post anestesia is not a cause of laminitis tho!!

Please provide the research links or citations for this.  The veterinarians here prescribe tramadol for dogs and I'm not sure why they would not know this unless the research is very recent.   I have my senior dog on rimadyl and that does appear to work.

Tooth breakage ??

I’m new to the group and trying to figure all of this out with uploading everything to where etc. but I really need a answer to something. I have a IR, Cushing horse who is on 1/2 tab of presend and 3 scoops of thyroid-L 2x a day. Ever since she was put on the presend and we were at 1/4 for months as she turned aggressive and refused to eat anything but her hay, she has broke three teeth in 5 months and two with in one month. She is 22 years old. She has been my problematic horse for the last year and recently in the last 6 months it’s monthly the vets have been out here at least once if not twice a month for something that is wrong with her. 

I feel like I’m doing something wrong with this mare but I’m doing everything I can for her. She seems as happy as she can be. But sometimes I wonder if she’s just not happy anymore. I don’t feel like she is ready tho either to be let go.
Im just having a hard time with all these problems she is having and how I can stop this or just make her comfortable and stop the nonsense every month

I just wanted to offer some support to you at this time.  My horse is on Invokana and has been for awhile now.  He would not be with me if not for this group and the invokana.  His insulin level dropped in a matter of days and he was more comfortable almost immediately and I too had done everything possible to get that insulin down.

This is the first winter since I have owned him that he is able to regulate his body temp, no blankets and no leg wraps needed... the difference in my horse is amazing...

I know you are waiting for your shipment to come and I know how waiting makes time stand still,  I'm not far from you, I can offer you a sleeve of Invokana 300 mg  until your shipment comes in and you can send me the replacement.  I'm happy to ship if off to you tomorrow - it shouldn't take more than a day or two (I know the mail sucks) to get to you?  Ship it back to me when yours arrives.  Let me know  my email timeinagain at verizon (dot) net

I gave Mill Equiox Saturday and Sunday mornings initially since she was limping so badly and she improved after that. Since her leg was so swollen this morning I gave another dose for a total of 3.

Hi Jenny,

Dewi looks to be a good weight but the lack of current bloodwork makes it hard to judge where he's at right now with regard to his IR being controlled or not.  Martha's already emailed you our welcome letter but to reiterate a few points:

Danilon and any NSAIDs are not helpful in metabolic laminitis and they can cause digestive problems when used long term.  You should probably start weaning Dewi off it as soon as possible to avoid any further issues.  Let us know if he is in fact still on the 3/4 of a sachet dose and if that's once or twice a day and we can help you develop a plan for weaning him off it.

As Martha noted Metformin has been found to stop working in some cases and since Dewi has been on it for 6 years and his last insulin test was well above normal it's very likely it's no longer working for him.  I'm not sure what the availability of Invokana is in the UK but if his diet is tightly controlled and his insulin is still high it may be an option if available.

Now, as far as his diet - the hay only needs to be soaked for 30 minutes in hot water or an hour in cold water to reduce sugar.  Soaking it more doesn't necessarily do any more reducing of sugar but it can effect other nutrients so I wouldn't go that far at this point in time.  While our general recommendation is hay under 10% ESC+starch we have a number of horses on the list who can't tolerate hay that high so at 8.6% if you're still using the same hay you had in August, I would continue to soak it.

The TRH stim test is one we don't recommend doing during the fall and early winter due to there not being an adjustment for the rise available for it.  Do you know what the pre-TRH number was?  That would be a better indicator of his PPID status than the 112 post-TRH number. 

His trim shows room for improvement  - he has a ski tip on the both front coffin bones and very little sole on either front foot so it's not surprising he's in pain without boots.  From looking at the pictures he's not wearing his hooves evenly (I would guess trying to compensate for the pain when he walks) and the trimming of the soles is not helping matters.  If you could post another note saying something like "Trim advice needed" to alert Lavinia she can comment more but I'd say there's nothing here that can't be fixed, it's just going to take a bit of time.
