You're thinking of post 250187. If the horse needs an emergency treatment give it, preferably with fluids. Stop Invokana until the horse has recovered. Fluids are very important for horses on this medication.

New ACTH and Insulin bloodwork

Last week I had bloodwork done on my PPID IR horse just as a check up. I had him on a lower dose of Prascend. So, I wanted to make sure blood levels were controlled. His outward symptoms seem to be very well controlled. His results on 4/6/2021 were:
ACTH 999 pg/mL Cornell
Insulin 122.71 uIU/ml Cornell
Glucose 108 mg/dL Vet's in house lab
G:I .88
RISQI .09
MIRG .10

I was very worried by these results and also confused as he seems to be doing really well. He often has "breakthrough" symptoms this time of year and he isn't this year and is shedding well and his thrush has been much easier to control, good energy, good appetite, no foot soreness. The only thing was that he is slightly heavier than I like him to be. However, I know why this is and he's already looking better in that department. So, I called Cornell (because my Vet isn't the least bit worried about these numbers) and they agreed that we should re run his bloodwork. These are the new results drawn 4/13/21:
ACTH 657 pg/mL
Insulin 87.29 uIU/mL
Unfortunately, my Vet would not re run the Glucose and even though I asked him to add it to the Cornell submission so they could do it, he didn't. So, I don't have a new Glucose.

Firestorm had been on a increased dose of Prascend for about 3 days when the new sample was done. He went from 1 mg to 1.5 mg. He got the new dose Sat, Sun and Mon and I drew his sample Tues morning. I wanted to get the sample on Monday to reduce the amount of the Prascend's affects, but my Vet would not agree to that as he wanted me to bring him the sample on Tues and I had no way of centrifuging it if I drew it Mon. The person I spoke with at Cornell said that Prascend starts to affect blood levels in 3 to 5 days. So, we will likely see some effect of the higher dose on the new sample. Only other difference is that Tues am was cool (about 45 F) and the original draw had been done on a warm afternoon and my Vet drew the first sample and I drew the second. Firestorm didn't appear to be stressed by the Vet's presence and we drew it at the beginning of the appointment, not after he had done a bunch of examining.

My questions are:
Is it silly to even do a G:I, RISQI and MIRG with the new results since the Glucose isn't from the same sample? His Glucose does tend to pretty much always be about the same every time I check it?

A Vet friend who knows what I'm dealing with and has seen Firestorm enough to be able to legally prescribe to him, sent me Metformin if I want to start it. Given the new results, should I still do the Metformin or would it be ok to just continue my changes (increased Prascend and hay analysis to tighten and properly balance his diet and start a regular light exercise regimen) and recheck him again once the changes have had time to hopefully have a positive affect?

Should I be concerned about any of the other abnormal levels in his CBC and Chemistry also run on 4/6? More details are in his case history, but he has elevated GGT and ALB and low Glob and PLT.

I've increased Firestorm to 1.5 mg Prascend and would like to recheck him before going higher. He's been well controlled on this dose and lower in the past and his symptoms are very well controlled. My Vet says I just need to go up to 2 mg. I'm not asking anyone to contradict my Vet, but if I want to try to stay as low as possible, am I putting Firestorm in any danger to do 1.5 mg and then retest him? When should I retest him to see if I do, in fact, need to increase my dose further? It's confusing because, on one hand, my vet isn't worried at all about these very concerning numbers, but then he wants to dramatically increase my horse's dose of Prascend. He said we can talk about diet in a couple of weeks. That seems like what we should be addressing immediately!

Consider speaking to your vet about trying a few days of Gastrogard to see if that makes him feel better. Have you been monitoring him with urine dipsticks?

I would go to free choice hay and the oats on days you work. Wait for blood work to see if Invokana may need to be adjusted.

New Insulin Test

As suggested, I had a new insulin test done for Nemo.  After balancing his hay, we made the feed adjustments recommended by Dr. K.  Our local vet said if his test came back normal, we could give him all the hay he wants.  He is a bit too thin right now.  I just uploaded his test.  He came in at 16.  Thoughts on diet change?  Would you agree to give him a lot of hay?  

Side note - his hooves are coming along.  We have another infection but are addressing it.  Finding a knowledgeable laminitis farrier made a world of difference there too.  To anyone looking, Kim Powers is amazing!  She was recommended by folks here in the group.

Thanks for your clarifications, Jane.

Another tidbit about TRH stim tests - for next time.  They are very useful for diagnosing early PPID cases.  Once the horse has been diagnosed and is on pergolide, just as Lavinia described with the fall testing, there are no reference values for a stim test with horses on pergolide.  We would compare the pre stim values from one testing to another but the post TRH stim values are not particularly useful.  I had not noticed that the testing was post TRH stim so the ACTH may not be as uncontrolled as I originally thought.

Hi Jane,

To add to what Lavinia has advised about taking hoof pictures while your girl is down , you will get a better view of the hoof (while you are doing contortions on the bedding) if you lay out a large towel or sheet under the hoof,in order to use this as background,as opposed to the shavings.
It's definitely doable with a little practice, and will give Lavinia something to work with to help her .

Hi hope everyone  is well ..checking in to date, over the past couple of months now after a very hard long painful season for Gavi we finally seemed to achieve a therapeutic dose of 5.3 pergalide compounded capsule.He is still acting pain free almost back to 100 %comfort but still mean to his herd mates alot of the time ,sleepy ,grumpy ,no interest in his etec mash with minerals and supplements. He only would like  his etec dry in handfuls ..kinda picky with hay and I have to Salt it so he at least gets that.Vet is coming on the 26 to repeat ach/stim test,check kidneys and triglycerides and insulin etc.anything else to check to know if pergalide and ivonkanna levels are on or off ?He is definitely not himself and I dnt really think he feels good.Tonight he was even acting like he was anxious,maybe peeing a bit more as well overall..TIA ,Tara
