Re: Ughh!!! Pilgrim confirmed to have PPID

So if you look at the Liphook chart (https://liphookequinehospital.co.uk/wp-content/uploads/Seasonal-Changes-in-ACTH-Secretion2.pdf) for week 37 he's elevated but not a crazy amount.  I would still aim to get him up to the 1mg dose sooner than a month from now though and plan on restesting as mentioned earlier.

Hi Jackie,

As far as boots I would go by what she tells you.  If she's comfortable on the footing that you have I would probably err on the side of no boots, but if she starts acting at all sore then I'd want something in the way of hoof protection (assuming it's coming from her soles).

I don't like Licorice myself, but had read that horses like it, so I ordered the Beet-e-Bites Pill Pouches in Licorice and my Icelandic just LOVES them!  After a couple of days of them, when I pulled the pouch out of the little bag, he grabbed at it and gobbled it down!!!

She is definitely a cutie for sure!  Thank you for looking at the pics. Her sole is very thin. My farrier said she may never grow it. That said, she seems to be comfortable and striding without discomfort. I want to start working with her soon, groundwork, but am concerned about her feet. Would you boot her?

Thank you, again Sherry!

hi.  i haven't been on here in a while and have been looking around.  it seems like some stuff is gone, like, for example, a list of lab tested food items.  is that the case or am i just not finding it?

Good job Jackie!  Her toes were definitely too long and not a lot of sole in July and she has some pretty significant sidebone in both feet.  She's very cute though!

Oh Kim I'm so sorry about the loss of the foal.

I'm mystified about the email issue and my only thought is that she should check her trash folder as well as the Spam folder but I just looked and it looks like she signed up with a different email about an hour ago.

I did answer this privately so sorry if it's a duplicate.

Your vet is correct to test at 7 days. There is no effect of stress on insulin in chronic conditions and the metformin effect won't get any better than it is at 7 days **unless** PPID is helping to drive it, in which case you will need pergolide on board (same for Invokna BTW)

Sugar is best tested when you pull insulin to see the relationship between the two but if you just need a baseline check any time 4+ hours after first meal of the  day.

It's not that NSAIDs are entirely useless just that they have minimal effect. What you are seeing could be from him being up and moving around for an hour. Try skipping it to see what happens.

Question about Seasonal Rise Coat Changes

Sarah Orlofske

Hello All,

I know I have to update my case history from our bloodwork in July, but it has been grueling at work and promise I will catch up soon - hopefully this weekend! But I have a question I am hoping to get some help with for Bullitt and Kate.

This is our first year monitoring for the seasonal rise. We had blood work in late July. Kate was at 50.4 pg/mL ACTH (Ref 2-35)  and Bullitt was at 32.4. Both were on 1 tab of Prascend. After the recommendations I received here, I gradually got them up to 1.5 tablets for Bullitt and 2 for Kate. I believe things are going well over all with their health, especially Kate who shows many more symptoms than Bullitt and those are now under control. However, Bullitt has never had any issues shedding out despite probably being PPID for a while. Kate has all kinds of shedding problems and we had to shave her this spring. Now I have noticed Bullitt is not growing in his winter coat at all and Kate has been, although it it is not outrageously wooly like it has been in the past. I am just looking to see what is "normal". We have really bad winters here and I am concerned about him not growing a thick of enough coat? Since this is our first year with them on Prascend I don't have a good baseline. Is it possible that Bullitt still needs to move up to a higher dose or is the dose too high and preventing the coat from growing in? Or is Kate needing a higher dose if she is growing in winter coat too early? These are our only two horses and I have been "trained" that minis typically look like sheep in the winter, so maybe I am just not used to what a "normal" winter coat is?

Any advice or recommendations would be greatly appreciated as always!

Hey Suzanne,

A product called Beet-E-Bites Equipockets works great for us.  My horse LOVES the peppermint and the apple flavors and they are manufactured specifically for IR/PPID horses so you know they are safe.    https://beetebites.com/

what's next

nikkibob1994@...

Darby is going through a laminitic episode, no change in circumstances. Negative for Lymes, ACTH is 28.8 and his last insulin was 50.
Darby started Metformin 3 days ago, still waiting for the Prascend (as per Dr. Kellon's recommendation). How soon should I be seeing an effect from it?, My vet said she wanted to test his insulin again in 7 days after reading on the ECIR site that we should. , but I thought it was not wise to test insulin when the animal is stressed? Which he is from his ongoing bout of laminitis.
I also bought a glucometer, but am unsure when to test, when is the best time to get the appropriate sugar reading?
I know it has been  repeatedly said that IR horses will not start feeling better until the insulin is controlled and that NSAID's like bute are useless. BUT, every day for the last week he has been unable to walk out of his stall in the morning until I have given him bute (2 grams) and within an hour or so he is visibly more comfortable, less stressed and happy to walk outside( from a deep bedded stall and he wears boots 24/7). Could this be something else I am dealing with?

There was no heartbeat on the ultrasound; they are aborting the foal.

Sherry,
I don't know how she signed up, because when she logs in it asks fo a password. She clicks the forgot password link, a box comes up and says an email has been sent to get a new password. She never gets an email.

Thank you, I hope I did this correctly. I need to name some xrays and delete duplicates. I will start a new thread but meanwhile, could someone look at her xrays when time permits? She has had 2 trims so far, backing toe.

Hi Jackie,

X-rays would go into a photo folder along with any other pictures you have of Rose.  To do that go to https://ecir.groups.io/g/CaseHistory/photos and click on the link that says "New Album" and name it "Jackie and Rose".  Please label any photos as outlined in the Wiki: https://ecir.groups.io/g/main/wiki/1482#Photos-and-Hoof-Evaluation-Help
