So if you look at the Liphook chart (https://liphookequinehospital.co.uk/wp-content/uploads/Seasonal-Changes-in-ACTH-Secretion2.pdf) for week 37 he's elevated but not a crazy amount. I would still aim to get him up to the 1mg dose sooner than a month from now though and plan on restesting as mentioned earlier.

Hi Jackie, As far as boots I would go by what she tells you. If she's comfortable on the footing that you have I would probably err on the side of no boots, but if she starts acting at all sore then I'd want something in the way of hoof protection (assuming it's coming from her soles).

I don't like Licorice myself, but had read that horses like it, so I ordered the Beet-e-Bites Pill Pouches in Licorice and my Icelandic just LOVES them! After a couple of days of them, when I pulled the pouch out of the little bag, he grabbed at it and gobbled it down!!!

She is definitely a cutie for sure! Thank you for looking at the pics. Her sole is very thin. My farrier said she may never grow it. That said, she seems to be comfortable and striding without discomfort. I want to start working with her soon, groundwork, but am concerned about her feet. Would you boot her?

Good job Jackie! Her toes were definitely too long and not a lot of sole in July and she has some pretty significant sidebone in both feet. She's very cute though!

Oh Kim I'm so sorry about the loss of the foal. I'm mystified about the email issue and my only thought is that she should check her trash folder as well as the Spam folder but I just looked and it looks like she signed up with a different email about an hour ago.

Your vet is correct to test at 7 days. There is no effect of stress on insulin in chronic conditions and the metformin effect won't get any better than it is at 7 days **unless** PPID is helping to drive it, in which case you will need pergolide on board (same for Invokna BTW) Sugar is best tested when you pull insulin to see the relationship between the two but if you just need a baseline check any time 4+ hours after first meal of the day. It's not that NSAIDs are entirely useless just that they have minimal effect. What you are seeing could be from him being up and moving around for an hour. Darby is going through a laminitic episode, no change in circumstances. Negative for Lymes, ACTH is 28.8 and his last insulin was 50.
I also bought a glucometer, but am unsure when to test, when is the best time to get the appropriate sugar reading? Darby started Metformin 3 days ago, still waiting for the Prascend (as per Dr. Kellon's recommendation). How soon should I be seeing an effect from it?, My vet said she wanted to test his insulin again in 7 days after reading on the ECIR site that we should. , but I thought it was not wise to test insulin when the animal is stressed? Which he is from his ongoing bout of laminitis. I know it has been repeatedly said that IR horses will not start feeling better until the insulin is controlled and that NSAID's like bute are useless. BUT, every day for the last week he has been unable to walk out of his stall in the morning until I have given him bute (2 grams) and within an hour or so he is visibly more comfortable, less stressed and happy to walk outside( from a deep bedded stall and he wears boots 24/7). There was no heartbeat on the ultrasound; they are aborting the foal.

Thank you, I hope I did this correctly. I need to name some xrays and delete duplicates. I will start a new thread but meanwhile, could someone look at her xrays when time permits? She has had 2 trims so far, backing toe.
