I have updated my case history and added some new photos as well, I did not add the new results since they have not been finalized. Thanks Sherry. I'm going to have to figure out the work thing for him. Right now because of elder parent care etc., I'm only getting out to the barn every Saturday/Sunday. I've been trying to go mid-week but that's been hit or miss. I'm going to have to see if I can hire our caregiver and/or our barn owner's daughter [though she may not be thrilled about walking him in the cold]..he can't be free lunged in the indoor arena right now because it's also doubling as a shelter for the farm's overflow of hay and unless he's on a lead he insists on "gait by" snacking. We'll figure it out. Have too.

Hi Katie,
His July insulin is typical for a well managed IR horse. Abnormal enough to confirm he's IR since he only had hay prior to testing, but well below the laminitic range. If he stays under 20 uIU/ml then he is doing great, and it's awesome you are being proactive and monitoring him now! Watch him as he matures and ages. Hi Lynn, I wouldn't be terribly concerned with those numbers right now but I would want to get him back into work to help with the insulin numbers as well as the weight gain. I'd also want to recheck him next year prior to the rise (end of June/beginning of July) so if the ACTH is creeping up you can get ahead of it prior to the rise. This is of course assuming that everything else stays good between now and then.

Hi Jessica, Was the bloodwork done fasting or non-fasting? She's definitely showing early PPID response to the TRH stim test but I think that based on her earlier results and physical signs she is IR but it's under better control now.

I have a questions about varying levels of ACTH, can that also influence baseline insulin levels? If baseline ACTH is wnl, would that correspond with possible lower levels of Insulin? And when ACTH levels are higher, could that correspond with higher baseline insulin levels?

Relevante Test Results Following Vet Visit - Question
I got Relevante's test results back following the vet's visit. Dr. K had recommended waiting until the vet visit and subsequent test results [ and to see if his foot soreness resolved now that we are coming out of the seasonal rise]. My vet agreed his foot soreness appears to be gone at this point and he said I could ride/exercise him. He did however say that although the crest itself appears normal in profile, when he put his hands on either side of the crest he said he still felt some thickness. And somehow the boy managed to gain 15 lbs. [not sure how that's possible as he is practically an air plant now]. I assume it's due to his enforced downtime. The vet also said to continue soaking hay.
Here are his numbers through the rise concluding with those just taken... ACTH June 2021 - 19.4 pg/mL Nov 2021 - 24.4 pg/mL Insulin August 5, 2021 - 44.74 ulU/ml August 31, 2021 - 18.71 ulU/ml Nov 2021 - 28.25 ulU/ml Both ACTH and insulin have gone up. Since nothing has changed in his diet [as we are continuing to soak hay] is it plausible to think that because his ACTH is elevated [does this mean it is not controlled?] that it's driving the insulin up? And should I consider adding a 1/4 of Prascend tablet to his 2 mg compounded pergolide dose to see if that improves his numbers? And if so, when should I test again? Or do I just continue to monitor and get the exercise going again? Hi Jessica,
My first thought is that I guess that addresses the question you asked, many posts earlier. We’re interested in her ACTH because high ACTH can result in high insulin, which can cause laminitis. Elevated ACTH itself does not cause laminitis. My horse, Logo, had PPID which was difficult to control but never did he have elevated insulin, nor laminitis. If Khan’s insulin is not elevated because of your management, then good for you. It’s important to keep it that way, but giving pergolide will not do the job for you. My second thought is that your case history needs updating, not only with the new labs but with everything else that has happened in the last three months. I know I have asked about the omeprazole and bute she was getting and you’ve replied but that should be a signal that your case history needs an update. I honestly can’t remember your answer. My third thought would be that it’s time to start a new thread on improving Khan’s comfort through booting and improved trimming. If you can’t find a trimmer who is willing to follow Lavinia’s advice, you could do it yourself, as many others here have done. I feel a bit weird suggesting this as I’m not one of them but those who have seem to make good progress. And my fourth (and final for now) is wondering what your thoughts are. -- Martha in Vermont

Andrea, is this a ryegrass hay? The only time I’ve seen WSC that much higher than ESC was with ryegrass. Also, does the lab report say what method was used to test ESC? Some members have found that NIR testing may return lower ESC values than wet chem testing on the same hay—so if your ESC was done by NIR, it may actually be higher than 8.5%.
