Hi Andrea,

Can't say much without a case history but I would be soaking his hay again at this point.  Do you have him in boots as he has not sole to speak of based on the x-rays?

Jessica,

You should start a new topic and call it something like "Trim & Booting Advice Needed".  You can do a trial of pergolide to see if it helps but if the issue is starting with the trim (which seems quite likely given her insulin level being well controlled right now) that needs to be fixed for her to feel better.  We always say this is a marathon not a sprint and there are no quick fixes.

Jessica,

There are several things that are not right and they're going to take time. Her hoof capsule is distorted from poor trim.  She has collections/abscesses to drain. Her metabolic condition requires long term vigilance. Since she is chronic there may be an element of neuropathic pain. All of this can be managed but it won't be overnight.

New Case Embarr

Andrea Thomason

All

please find Embarr’s case history, blood work and X-rays.  He’s struggled from he turned up in the UK 2016.

We went down the ulcer and PSSM route and he’s progressed into a IR case and struggling with RF lameness.

We have sources 10 acres to grow our own low sugar meadow hay and the attached analysis is the first crop.  On paper the ESC and starch looked good and we slowly transferred to dry and he’s lame again.

I will upload his diet - any help and advice would be greatly appreciated as Embarr on a off day is sadly dangerous.

Thanks in advance

Do I start that as a private thread for improved trimming and booting?
I have updated my case history and added some new photos as well, I did not add the new results since they have not been finalized.
My thoughts: I'm completely deflated that prascend will not help her.

Non-fasting. But she is in more pain again - something is not right.

Re: Relevante Test Results Following Vet Visit - Question

Lynn

Thanks Sherry. I'm going to have to figure out the work thing for him. Right now because of elder parent care etc., I'm only getting out to the barn every Saturday/Sunday. I've been trying to go mid-week but that's been hit or miss. I'm going to have to see if I can hire our caregiver and/or our barn owner's daughter [though she may not be thrilled about walking him in the cold]..he can't be free lunged in the indoor arena right now because it's also doubling as a shelter for the farm's overflow of hay and unless he's on a lead he insists on "gait by" snacking. We'll figure it out. Have too.

Hi Katie,

His July insulin is typical for a well managed IR horse.  Abnormal enough to confirm he's IR since he only had hay prior to testing, but well below the laminitic range.  If he stays under 20 uIU/ml then he is doing great, and it's awesome you are being proactive and monitoring him now!  Watch him as he matures and ages.  He's still young to see problems with IR but after about 5 years old it can start to cause issues.

Re: Where to add info on hay analysis and how to find my case history to add in blood work results and a few more questions

Dancer's Mom

The hay analysis was done via wet chem. I’ll try to update Dancer’s case history with your suggestions. Thank you for that.

Also, I was going to use Sand Clear or a product like it, to aid in fecal sand removal. I usually give that type of product to Dancer once a month. But since she has EMS and is IR and has Cushings, now, I wanted to check with you to see if it’s ok to do that.

Thank you so much.

sincerely,
Patti

Hi Lynn,

I wouldn't be terribly concerned with those numbers right now but I would want to get him back into work to help with the insulin numbers as well as the weight gain.  I'd also want to recheck him next year prior to the rise (end of June/beginning of July) so if the ACTH is creeping up you can get ahead of it prior to the rise.  This is of course assuming that everything else stays good between now and then.

Re: Is it safe for Khan try prascend/pergolide (Dr. Kellon please weigh-in thank you)

Hi Jessica,

Was the bloodwork done fasting or non-fasting?  She's definitely showing early PPID response to the TRH stim test but I think that based on her earlier results and physical signs she is IR but it's under better control now.

Thanks Maxine,

I uploaded a photo. It's at https://ecir.groups.io/g/CaseHistory/album?id=270402

I have not created a case history for him since he's not currently IR or PPID.

Thanks Jill - They do look similar.

I have a questions about varying levels of ACTH, can that also influence baseline insulin levels? If baseline ACTH is wnl, would that correspond with possible lower levels of Insulin? And when ACTH levels are higher, could that correspond with higher baseline insulin levels?

Hi Nancy - Can you please send me the link to change my credit card settings for my monthly donation? I've searched and can't find it. My credit card had a fraud alert and I need to change # to the new card. Thank you in advance...

Dear ECIR Group Members --

If you’re reading this, you have likely benefited from the free services available through the ECIR Group Inc. Maybe you received direct help from one of our volunteers; perhaps you discovered and implemented useful protocols from our files; or maybe you learned valuable information by following the story of other members and their equines.

Everything you have learned from the ECIR Group Inc. is a direct result of the work of our hard-working volunteers. I’m blown away at the fact that the volunteers here on our free outreach group have handled an average of 1,122 messages PER MONTH this year! These messages come from and go to people in 33 countries seeking help for their horses, donkeys, ponies and mules. What might not be as evident is the work that goes on outside of this free outreach group. One of our largest projects this year was putting on the 2021 NO Laminitis! Conference.

Even though we are an all-volunteer group, there are costs associated with what we do. That’s why I’m writing to you to ask you to consider a small monthly donation to the ECIR Group Inc.

To those of you already donating monthly, we are most grateful. We’re also most grateful to those of you who make one-time donations, either end-of-year, in honor of a beloved equine, or because you feel moved to help. If you’re not currently donating to ECIR, I hope you’ll consider making a donation now. Whether it’s $5/month or $50/month, no amount is too small (or too large!). We’ve only retained 35% of the donors we had in the beginning of 2021, so we definitely have some catching up to do.

Giving Tuesday is today, so there’s no time like the present to pledge your support to the ECIR Group Inc. Our ultimate goal is very personal to most of us here – the prevention of laminitis. Through hard work and education, we are making progress! If you’d like to make a tax-deductible donation, go here: https://bit.ly/2J5xhKo. To make your donation monthly, please click on the monthly option.

On behalf of the ECIR Group Inc. Board of Directors, thank you so much for your consideration of this request.
