Hi Angela, 

Here in Canada we can't get APF to help our horses adjust to pergolide so both my vet and ECIR recommend starting with 1/4 pill for the first few days, then increasing by 1/4 until the full dose was reached.  I definitely noticed some minir inappetence and lethargy, but that passes once they adjust to the meds.

Answer re: symmetry - muscle wasting is not perfectly symmetrical but definitely on both sides and fairly equivalent. Its most pronounced in the croup, but also top of neck and around gaskin, the rest I think is just being underweight.

Answer re: insulin – was in the low 20s in January (non-fasting Cornell test), my vet gave me that via paper and I am terrible with paper, need everything digitally.

Answer re: video – of course, it may just take me some time as I need a helper to get a good video.

Answer re: test – I don’t know what 8 means either and my vet is not an expert so she is consulting with the SME on context and treatment, I also don’t know why they highlighted retest titers in 1 or 3 months as that seems like it is designed for retesting of treated horses but still waiting to hear back on my vet’s consultation with their SME on the test results/treatment. The test is done via blood draw.  I had the test run because Khan has been pretty much unresponsive to the ECIR protocol, she slightly reminded me of EPM horses I have seen and every now and again she has movement abnormalities that are not severe but mildly similar to neurological horses.

Answer re: Lymes – Khans Lymes test was negative, I need to get it from my vet they just called me on that one.

Any thoughts on Relyne GI

Good Morning,

Wanted any input on using Relyne GI for my IR/PPID mini. He has been on maintenance dose of Ulcergard for many months and fingers crossed has not a reoccurence of ulcer symptoms. His last fecal blood check was negative. Scoping is not possible according to my vet. My vet has also said that previous fecal blood tests reveal gastric and colonic ulcers. My vet has recommended the Relyne product but doesn't provide an explanation as to why other than it is what he has his clients use. Is it safe? And is it effective? If I change over to this product, do I stop the Ulcergard cold turkey?

My increase for a 90 day supply of 4mg capsules was only $30. So I went from paying $180 to $210. I definitely can't complain about a $10 increase each month. Still confused about others having an increase of $50+ for a 1 month supply.  Hope they are able to figure things out and help people out.

Alfalfa can cause issues with some horses even though the ESC+starch is below 10%, that's why we don't recommend it for IR horses.

As Amex is still overweight it's not really surprising his insulin is still elevated. I would recommend continuing on the diet plan (no more than 22lbs a day total and possibly less if he isn't losing weight on that amount) and using correctly dosed Metformin to see if that helps with the refractory insulin.  As a group our members have not found Insulinwise to be a useful product (although I believe they offer a money back guarantee if it doesn't work for your horse).  

Glad the new vet is on board with the Prascend probably not being necessary.  You don't really need to wean it down but if you're more comfortable with that you can follow that plan.

I too would love to see a video of Kahn, I had never heard of this and really couldn’t find much to read up on except one study that I didn’t enjoy.
Did you have a vet familiar with this? And was the test done by muscle biopsy? I’m just wondering how you came upon this in particular?

Sorry for all the questions I’m just really curious.

1-Yes I am feeding Alfalfa Cubes (since last week, 8 oz/day) as a carrier for the California Trace Plus Pellets & Ground Flax. He didn't eat the Dehy Cubes for some reason or Beet Pulp. The Nutrionist gave me the ok on the alfalfa cubes :-/    I read up on it too and it everything I saw said it was safe for IR horses. You think that small amount is bad for his Insulin or is it the Proteins?
2- No weight scale at the vet. His current weight is 1,161 (weight tape per specified math)
3- Yes his insulin was tested before the metformin was stopped. Since I stopped working with the hospital (who under dosed Amex) and started working with a new field vet service (she said 16 2x a day would have been the correct dosage). I can put him back on the metformin before the next the next blood draw on 3-15-22.
4. Hospital vet was the one who kept increasing the Prascend; eventually getting Amex on 3am/3pm. My new field vet asked why he was on it on the first place (and why the am/pm & why such a high dosage). She recommended us weaning him off and rechecking him in 4 weeks to see if his ATCH numbers are truly out of range. No real change in his ACTH with history of 1 Prascend to 6. I didn't stop the Prascend before the vet visit on 2-8-22, we talked over the phone 2-3-22 and made that plan before she came. By the time she came and made her notes he was down to 3am only. {Weaning off from 6/day was...3am for 5 days, 1 am for 5 days, then 0}
4-Got it. I did the new thread for Lavina. Farrier is coming Tuesday 2-22-22. The newest xrays are really indicative of his long toes. Glad I got the hinds snapped too. Way too long as well. On it.
5-Continuing with 1 hour soak and a Rinse his hay (not doing that 4 hour suggestion). Not doing Safe Starch either. Maybe I will try the Stabul if your concerned with the 8oz Alfalfa Cubes.

Still confused as to his super high insulin when his diet has been in check for almost 2 months. I may give Insulin Wise a try.

Thank you Dr Kellon for reviewing Gilligan's case history.  Re:  CBC results - he has had low values since he was about 8 years old and showed the same lethargy.  I do not have those test results (pre-digital age and that vet has left the practice).  About his feet - when we bought Gil at age 6, he was shod with pads all around and very shelly hoof walls.  We were told he was prone to stone bruises without the pads.  We had the pads removed after a couple of years, and he did indeed get stone bruises. His shoes often came loose and off due to thin hoof walls. About 10 years ago, we removed his shoes and I took training with an AANHCP certified natural hoof trimmer, who mentored me for 6 months after the classes. Gil's hoof walls improved dramatically in thickness and durability but his frogs have remained wimpy.  I take very little off the soles which have also remained thin. When trail riding in rocky terrain I boot his fronts. I will study Pete Ramey's info on Distal Descent.  Thanks so much for your help!

I didn't feel that the Avrio price increase was too bad.  I think mine went  up $14 from the Pet Health price.  I just placed a reorder with them.  They said they would contact my vet and I also contacted my vet to give her a heads up.  They contacted her and she okayed it and I got the Pergolide in two days!!!  I was amazed at the quick service!  Especially that USPS managed to get it here that quickly...
