Once you have your test results back you'll have a clearer path forward.  Did you have them do a TRH Stim test as suggested by Dr. Kellon (https://ecir.groups.io/g/main/message/277388) to see if PPID is possibly an issue or discuss Metformin as Dr. Kellon suggested here: https://ecir.groups.io/g/main/message/277358

Is there a reason you had her insulin redone today since you just had it done a week ago?  Unless you changed her diet the day the blood was drawn it's unlikely to be a different result now.

Dr. Kellon, I was giving Dually the Strongid C2X for one month to get tape worms. Since I am giving him the Quest without praziquantel, can I continue the C2X?

I just got the blood results. Triglycerides are 1.97 mmol/L (reference range 0.09-0.58).
Is this very elevated?

Soaked Timothy (hay analysis is in my photo album). She gets 3 flakes a day in a hay net (not sure on lbs). She also gets 1.5 cups (before soaking) of the Ontario dehydrated timothy balance cubes with flax, J-herb, and MadBarn Aminotrace. She doesn't really like this though (she has never been a fan of beetpulp or hay cubes), so sometimes she eats it, and sometimes she doesn't, or she'll just eat some of it.

She's a really good weight (with ribs slightly visible and the fat pads on her rump are gone, and the ones on her shoulders are getting smaller. I think her crest is maybe even starting to get a bit smaller too). 

I doubt she'll eat L-carnitine. She's a picky eater and its hard to get her to eat anything consistently (unless it's really high in starch/ sugar).

My pony is currently on Metformin (with uncontrolled insulin). I've been waiting on the blood results to see about starting Invokana, and unfortunately they just came back with a high triglyceride result (I don't know the exact number - waiting on the test to be sent over to me). Because of this my vet doesn't want to start her on Invokana for fear of causing liver and/or kidney damage (apparently he talked to a vet who is involved in a clinical trial with Invokana in horses, and he recommended that my pony not have Invokana as well). 

Definitely disappointing

Stop the Tylenol first, cold turkey, and observe. Then stop the gabapentin cold turkey, observe.  There's a good chance you won't see any significant difference at all. When down to Banamine only, start the  PQ and begin the taper.

That's called banking the stall and yes, the  bedding is higher along the wall and slopes down toward the center. You really need to get your vet involved at this point. He/she can do a neurological and lameness exam, check the retinas and the very best thing for anxiety is diazepam (Valium) which could break this cycle.

If he is not having problems there is no reason to supplement for gut. He definitely doesn't need Vermont Blend with the cubes.

Tri-Amino can't make up for a deficit in total protein. The 50:50 mix should be adequate so just give it a little time. Yet, you can give him the Quest but no need to continue the daily Strongid if you do.
