Hi, Rene. Definitely check ACTH, insulin and glucose. You want to evaluate how any change in PPID is affecting your ponies' EMS. To do that, you need insulin and glucose. IMO this is perfect time of year to check that your pergolide dose is controlling PPID. I like a head start on the seasonal rise that starts in July. If you need to bump up pergolide dose, you have time for the increase to kick in and to make a plan to boost pergolide during the seasonal rise.

Hi, Lisa. Don't be discouraged. Nira has a way to go, and it requires the patience of a saint.

I have a question on your CH that mentions liquid compounded pergolide. Can you tell us a little more about this formulation? Is pergolide in water or oil solution? Back in the early days, CP was sold as a water solution. Any non-molasses beet pulp is fine.

You'll need to rinse/soak/rinse any variety to remove dirt/iron.

Hi Andrea,

Glycerin is used to male the soft gel so is an ingredient in virtually of all of them. It's not a problem. Just need to use ones that include some type of oil inside the gelcap (soy is the most common).

Hi, Laura. 

To update your case history, you need to edit the original Pages.app word processing document that you should have saved on a computer hard drive or in the Cloud. Editing the PDF that you uploaded to Groups.io requires special software. 

To edit Captain Drifter Case History.pages, open it up using Pages.app. Be sure to change the date of the update on Page 1 and then add the blood work and any comments. You have plenty of room to add new labs without editing the tables. Then change the document name to Captain Drifter Case History 2022-05-28.pages or something similar to show the current date. Save it on your hard drive or in the Cloud. Export a PDF of the updated CH and upload it to your CH folder linked in your automatic signature. Give a shout if you run into problems.

Just wondering if ACTH is the only thing that needs to be retested when we do test after reaching the full dose for 3-4 weeks? No insulin levels or glucose?

Yes Nancy, Scotty has been on 2,000 iu Natural (in oil capsules) Vitamin E for many years. So sorry I missed including that in my case history, but I have remedied it now. Thanks for pointing it out.

Kristie - I have never had any luck with the vinegar/soap solutions, even buying the strongest vinegar I could find...well over what others have mentioned. The weeds just lapped it up and said "give me more."

Last year I bought a burn torch that is meant for weeds. That is the only thing that knocks them down, but it was a huge job the first time, and now I see I need to get out there sooner rather than later to start over again. We ended up buying another one so my husband and I could both work on it to make it go faster.

Captain Drifter update

Hi all,
I finally got follow up insulin results for Captain Drifter. Its still >200. He started Metformin March 4. He slowly responded to it, and on April 18 got the Ultimate shoes removed and got into boots. He relapsed May 1. My excellent trimmer comes every 2 weeks. My vet drew blood April 1 to check insulin/glucose but the sample was lost and weeks went by with my vet saying no results yet. Blood was redrawn with results arriving May 24 showing insulin >200. No mention of glucose. He’s on custom mineral blend and is eating the recommended amount along with Vitamin E gel caps with 4 TC balance cubes soaked for dinner.  The custom mineral blend is in a flax base and I was told no need to add flax. He gets Uckele J-herb 30 minutes before hay in am and 30 min before dinner. His gums are nice and pink. He gets 10 lbs weighed hay with ESC + starch well below 10%. He won’t eat it soaked. He’s in a dry lot of 8 stalls joined with 1-2lb hay bags spaced around. I asked my vet today for Invokana and he said there wasn’t much else we could try. But he’ll do some research. I have photos my trimmer took - I’ll try to get them uploaded. I’m having trouble getting the blood work updated- can’t figure how to edit. I added the pony’s TRH Stim test but put it in the wrong place. He doesn’t have Cushings. Thank you all!

I'm discouraged. Is the sinking with the rotation something she will be able to come back from? Will I be able to ride her again? 

The blood tests are back. The pre-TRH ACTH level is 25 (last time was 15.5) and post-TRH was 276 (last time was 188). 

Her insulin level is 32.5, which is down from the 42.5 last time. 

I am using .5 mg of compounded pergolide. Should I increase the dose? Do I need to switch to Prascend? 

I really would like to stop using the Metformin, but I am guessing that is too soon with her insulin still at 32.5.

I managed to get a glucometer reading today at the blood draw (***trick with the glucometer is to keep it and and test strips in a warm pocket next to my body until right before use and use a drop of blood straight out of the vein without any cooling or preservatives***).  Glucose remains high at 113. I hope that's from testing during the first morning feed and the stresses of laminitis pain. Her former glucose levels were commonly around 80 when taken 4 hours after morning feed.

I'm having trouble arranging a urine sample to check glucose clearance. Her urination pattern has changed, more frequent urination in smaller amounts, so it's hard to stage conditions for a catch. I walk around with a test strip in my pocket but can't deploy it in time.

Summer is schedule for surgery next Friday.  The Dr I spoke with was very informative and mentioned a small risk with a horse that had IR that had an ovarectomy that developed laminitis after surgery.

I am wondering if there is anything I can do to lessen the risk albeit small?

I did do a small bump of pergolide from 2.4mg to 3.0mg because I noticed some edema just near her teats and she paused shedding.  I am contemplating another bump to 3.6mg.
