Vinnie and Invokana update 6/2022

Nancy & Vinnie & Summer

Hi Dr Kellon,

Final labs came in....drum roll....

Lab Reference Ranges:
Insulin: 10-40 uIU/mL
Glucose:  71-122 mg/dL
ACTH:  2-35 pg/mL
Tryglicerides:14-65mg/dL

VInnie's result
TG 568, was 307
Insulin 30.9 was 103
Glucose: 103, was 121
ACTH 15.3, was 25.3

His diet consists of:

18 lb 9.9 eac + starch hay free fed
8 cups cavalor fiber force, 2.5 lb stabul 1 mixed in a bucket and free fed.
10 cups beet plup dry weight rsr mixed with 8 cups TC Senior split into two feedings. (note this was reintroduced this week he had stopped eating beet pulp with the neighbor fiasco)
4lb dry TC balance cubes thrown in tub with freed fed cavalor and stabul 1.  He snacks in this all day long and eats most of it in a 24 hr period.

I have ordered the pea protein isolate from Bulk Supplements ans plan on starting 100g when I get it this week.

Overall he is going great. He has been a little dull at times in the last week or so, but we hav had alot going on with the new neighbor mare ans then Summer leaving for her ovarectomy and coming back home. I think he was worried about her.

He is a little ribbier than he was, tape weight shows he has dropped a little from consistently 985 to now consistently 953.

Let me know what you think about TG and when to retest.  I am thrilled with these numbers!!!!!

TG is the last to overcome :)

Re: One month post-laminitis status on Cayuse

I've been rinsing, not soaking, her 2021 WFC August blue hay because I'm afraid of removing too much ESC. She's been eating the same load of hay since August. In an abundance of caution, I've been incorporating a no-nitrate hay, Diamond's hay, https://ecir.groups.io/g/CaseHistory/files/Cass%20and%20Satra%20-%20Cayuse%20-%20Diamond/Hay&%20Pasture%20Analysis/2022-04-13%20White%20River%20603+Cl+S+nitrates.pdf   as half her hay over the past week and a half. During that time her hoof pain has gotten steadily worse.

Thanks. That's all useful. I do have Arnica and I can try icing. I'll try again with my vets.

Need assistance with trimming hooves and BCS

a.k.a.petpalace2@...

Uploaded photos and X-rays .  No recent x-rays just ones taken last fall when acutely laminitic. Also have one body shot from that time and well as 2 recent ones, about a month apart for body condition scoring.

I mistakenly put same LF Dorsal hoof photo in twice but don't know how to delete it.  Can you explain how or can someone do it for me?

Thanks

Maybe you could put a thick sponge in the bottom of the bucket for him to stand on

Hi Robin and all

Robin, I know you have a lot on your plate.  Hoping I can please ask you to start new subject line when the general topic of your post changes.  In this case you have moved to radiographs and pain/abscesses and I have changed your subject line to that topic.  Yesterday I changed your topic to metformin.

If the topic continues here fine.  If you need to address a new area, please change the subject by starting  new topic. You can find that link under NEW TOPIC on thee right of your large device screen or, if on a smaller device, it is listed under the stack of coins icon at the bottom of your screen.

This may seem picky and it is, but with nearly 100 messages in your initial thread of Laminitis Spike, it will help your volunteers find and review what is going on with your boy more quickly when needed. Just go to the NEW TOPIC link to start a new are of discussion.

Many thanks indeed for helping us help you.

Robin, you can just poultice if soaking is causing that much discomfort. Standing in a bucket means he has to stand on a hard plastic or rubber surface of the bucket and when abscessing, can be very painful as any hard surface pushes on the abscessed area. I use Numotizine in a disposable diaper. Just pile a glob of it in the diaper, then put his foot in it ( do this on a soft surface such as a stall floor with shavings, then use the velcro tabs of the diaper to close it.  You can put a boot back on or what I typically do is make a boot out of duct tape( Gorilla tape works really well). Let me search for the video I learned from to make the boot. I then apply that to the bottom of the foot over the diaper and bring it up on the foot to stick it, the video will show you how to do this. I find duct tape boots often last two or three days and the Numotizine stays fairly soft. It has some analgesic properties also and it helps take the edge off the pain of an abscess. I had to go to duct tape as all of my abscess patients in my barn HATE boots. I'll post the video in another reply.  Also, the video shows a perfectly normal horse standing on a pristine cement floor holding it's foot up for minutes. In reality, your boot will have hay stuck in it, you will have numotizine on your hands, pants and shoes, and your patient will have snatched their foot away numerous times. Don't feel like you have failed, you're working in tough circumstances and things won't look perfect. But the boot will be on and the numotizine will help tremendously.

While his left fore is worse, his right fore is also very painful. I find when picking up his left fore to place in a bucket to soak then back into the boot, he is falling down on the ground and fortunately he is small and my husband helps me hold him up. Also, standing without a boot in the bucket to soak is causing him a lot of pain. Does anyone have any suggestions?

Re: Superman's weight

Anette

I wanted to have him checked in Feb. but the vet broke his foot and couldn't make a farm call and apparently the other vet couldn't  see him because he's not her patient. If he's going to get checked it needs to be soon to avoid the start of the seasonal rise, correct?
I remember it being mentioned that if one knows how to draw blood they can do it as long as it's spun within 4 hrs. I'd like to do that, but does it need to go into a certain tube? Would my vet have issue if I asked for one? Wondering if others do this how they approached the vet?

Are you seeing any correlation to her cycles or any time frame repeating for these flares? Does her crest change when she's more painful?

Could you put her on straight ODTB or TC Timothy Naturals at least short term to see if that helps?

You don't have to have full blown nitrate poisoning to have hoof pain. Are you soaking?

Re: Can icing relieve pain of Lyme or hormone-induced laminitis?

Icing can have short term pain relief for any kind of pain. It's most likely to be effective if her feet are hot. You can put the ice into sandwich bags and wrap them around her hoof and pastern.

Your vets don't need to stock doxy. They can call a prescription in to any human pharmacy for you. If it's only a short time, say less than a week, it shouldn't have a major effect on the Multiplex.

What about Tramadol? You can also try OTC lidocaine cream over the back of her pasterns and coronary bands, or an Arnica cream like Zim's Crack Creme or Arnica 35.

Can icing relieve pain of Lyme or hormone-induced laminitis?

I’m looking for short term pain control for Cayuse’s laminitis while we await new blood draws. Is there any harm to trying icing? She didn’t  eat well last night or this morning, down in her stall, bounding pulses in front. She’s swaying side to side in front and now her hind end seems very sore, too. She’s reluctant to lift her feet, whether to walk or to boot. Icing isn’t going to be easy.

I can press my vets to trial doxycycline tho I don’t know if they routinely  stock it. Will treating now invalidate the Lyme multiplex? I didn’t get a clear sense from the Cornell website.

I’m sorry, I did not take pics of my farrier measuring the depth.  She measured and I took notes, so I only have the measurements of the depth listed in the sole length photos.

Then I need your help! I see hoof wall measurements and sole length measurements. Can you link the photos showing the depth of the collateral grooves?
