Try getting one of those thin flexible plastic cutting boards from Walmart and cut it to the shape of the bottom of the muzzle. Slip it inside--problem solved--no hole! Here's an example,

My pony is a Houdini at muzzle tape,
I make a huge X with gorilla tape ,( stickier than duct tape), so that the x goes up the webbing on the sides a bit then run a band of tape around the whole

I am not a vet , but have just dealt with abscesses with my pony and one of my non IR Arabs just recently
no antibiotics were needed with either horse, my pony was painfully lame for 5 days

The usual precaution is to avoid J herb within 24 to 48 hours of surgery...due to clotting time.
Based on Justin's previous sedations- your vet will know what drugs are appropriate. My vet

You are right on all counts. Thyroid meds won't cure IR but may temporarily help to jump start a sluggish thyroid if the horse is overweight and thyroid numbers are lower due to primary
