To be clear, she's not currently on anything. I hadn't had her tested since 2016 since she was never symptomatic - lesson learned. I imagine I need to get her going on something?

All you know at this point is that her current dosage is not enough to control the rise numbers which, as you have seen, can be quite dramatic - especially in a horse with PPID. Your feeding

The seasonal rise is a normal physiological phenomenon which temporarily releases control on the pars intermedia probably as a secondary effect to hypothalamic changes needed to trigger the winter

yes I have xrays but they are not current and from 2018 as well as back when she had her DDFT injury. They are under her hoof album and I will get these moved over to one album soon.

Do you have radiographs? Doesn't sound likely this is laminitis. If you are going to stop or reduce pergolide I would wait for November.
The small bump in insulin could be related to weather.

Whole oats going right through? Check the manure, pull out the suspected whole oats, squeeze them open to see if the kernel is still there or is it just an empty hull. Appearances can be deceiving.

Whole oats is fine. Research has shown processing does not improve the digestibility of oats, unlike other grains. The hulls aren't even fermented well so they so up in the manure - but empty. Oil

My experience feeding straight oats goes back to the dark ages, but at the time crimped were recommended for the horses to get the best nutrition as the whole oats were expected to go right
