That hedgerow access probably isn't doing her insulin any good.
Has her diet been analyzed for adequate iodine and selenium? Those two minerals are critical for thyroid function.

Sorry, I’ve been overwhelmed at work…
Tartine doesn’t get meals per se. She has available soaked hay 24/7 because she is on a track with 4 other EMS horses. The hay is available in slow feeding

At 22yo, testing for PPID and EMS/IR certainly makes sense, esp. if the horse is exhibiting some unusual (for it) symptoms that can't be easily explained in other ways.
