Hi Laurie,
If nitrates are elevated, soaking the hay does help lower them. You can also mix it with another low nitrate hay.
A BCS between 4 and 5 is good. I would try to maintain that and taper off

If only three weeks you can probably just switch. Use a full dose of PQ pellets twice a day and you can even double that if needed.

It's always the lower the better with ACTH but I think the 0.25 increase during the rise period should cover you. L-carnitine should be 1 gram per 100 lbs bodyweight and you can give that twice a day.

Your basic numbers say that Tuck's insulin could be better controlled. Whether that involves tightening up his diet, increasing medication or something else is going to be impossible to say
