From what you have described, if he were my donk, and the range were 9-35, I would be starting him on pergolide.

Laminitic episode!?
I’m with out anymore options bf the vet call and I believe I have gone thru everything. My mare has been walking like she has 4 peg legs for a few days now. She is everything (IR,Cushings,etc) her

Laura, some people do decrease or eliminate the pergolide after the seasonal rise if their PPID is not adequately confirmed . You can try that and retest after three weeks but I don’t think it

You are correct that the ESC and starch are important information to have. The chopped oat straw can also be problematic, so unless you have an analysis for that it would be wise to stop feeding
