Hi Olivia,
Her current insulin level is not in the danger zone but I would definitely keep her on a tightly controlled diet and exercise her as you can.

She is grade and of unknown background. If it helps, we also elected for a basic blood test (like a CBC?) when we did her insulin at baseline. I can add that to my photo

Jiji was diagnosed with Lyme in 2020 with the assay from Cornell. It was apparently a chronic case, my vet said. I wasn’t keeping track of her records then, so I dont have bloodwork results from

The vet gave him a body score of 4.5 last March, and called him a "little thin"
I would say he is a 5 now. You can't see his ribs if he is just standing square, but when he does most

I feel like I'm missing something here. An insulin of 36+ is clearly abnormal so she is IR. Do you have labs past the ones from 2021 in her file? The bulging eye fat is characteristic of EMS but not

Bulging eye fat can be normal for overweight horses. But we can see her ribs, so she's not overweight. The hard fat bulges above her eyes are puzzling and I would not dismiss that observation. It

A trial on 1 mg pergolide is a possibility, but it's a bit late to start now. Doesn't mean you can't do it anyways, but you might not see the full effect of the medication.
