I will start treatment soon and know that I have to start slowly. My original plan was .25 for three days .5 over three days .75 for three days and remain at 1.0 and see what happens. My question is

It sounds like uncontrolled PPID with exacerbated IR (could be made worse by cold weather stress) is the most likely issue here. Instead of muscle enzymes I would nerve block the feet. If that

Sorry to hear Pauli has had a setback.
When was the most recent blood work done? The ACTH is at the top of the lab's normal range so it is possible there is a PPID component here.

What have the temps been doing in your area? Cold snaps can cause insulin spikes.
Iron overload and diseases such as Lyme can also cause this.

I have 2 horses clipped in winter yearly. One is an icey...he gets fully
clipped twice yearly usually but this year since he is laminitic and we
can't ride we trace clipped him. We never blanket him

I read that a pill can be put on a grape or hollowed out carrot. I do that as a treat and pil time stopped being a struggle.
