I agree with Lavinia that she is likely not IR (as you suspected). The low glucose is probably due to sample handling problems, which could also mean your ACTH is lower than actual but at 7 years old

The blood work was taken before breakfast (she generally gets fed around 9:00am, blood was taken at 10:00am) but she had her hay and was out grazing overnight, so I don't

I would think it would lose potency as you are supposed to refrigerate the pills. I would also worry if your horse is getting the entire dosage if you sprinkle it on his pellets. Just some thoughts.

If you are using Magnesium Oxide for the Temporary Emergency Diet, then you would feed 1.5grams/500lb.
That would be 3gm...or 3,000mg.
Giving 420mg tabs, that would be 7 tabs= 2,940mg.
