Sorry to hear Pauli has had a setback.
When was the most recent blood work done? The ACTH is at the top of the lab's normal range so it is possible there is a PPID component here.

What have the temps been doing in your area? Cold snaps can cause insulin spikes.
Iron overload and diseases such as Lyme can also cause this.

I have 2 horses clipped in winter yearly. One is an icey...he gets fully
clipped twice yearly usually but this year since he is laminitic and we
can't ride we trace clipped him. We never blanket him

You an do either but adding Cubes is probably the easiest.
In he FILES look for cold weather feeding chart that helps you determine how much more you need to feed for cold weather.

I read that a pill can be put on a grape or hollowed out carrot. I do that as a treat and pil time stopped being a struggle.

Finally found a way to administer Pergolide
I am finally able to get the Pergolide tablets down Beatles throat without him spitting them out or clenching his teeth. Perhaps others have thought of this before me. I saved the plastic "syringe"

I read that all horses with Cushing's should be clipped even in the winter. I also read about a low trace clip. If this horse lives in San Diego where to a Weather during the day in winter could be 50

adding BP for winter calories
My elderly pony Ginger gets only ODTB cubes but is ribby. She's about 450 pounds (guess) and gets 9 pounds cubes.
Could I add anopther pound of cubes for the winter, or add more BP ?

Rice bran is not a good idea for an IR horse. Way too high in fat of the wrong type as it has an inverted omega 3 to omega 6 ratio.
First step would be to give him extra hay. Then adding

Thanks for the help with the psyllium. Now I need to know if I can feed any rice bran to my IR/Cushings 26 y.o. MFT gelding. He's been ribby all summer. Now It's a lot colder out.
