Just to be clear, there is no significant bony rotation - movement of the coffin bone out of alignment. There is capsular displacement, movement away from the bone column.

If I had to pick between TRH stim and Lyme, I'd go with Lyme. Insulin should be repeated. The relatively mildly elevated insulin and glucose could have been a stress response since they were pulled

I was just rereading everything and the Lyme would certainly explain pain out of proportion to the insulin and radiographs. In fact, that's what we very often see with Lyme. I would suggest a Lyme

Haven’t had any bloodwork done since her episode; we are scheduled to have the primary vet out on June 30th for insulin and TRH stim test.
We had also considered doing another Lyme test and a biopsy

Any horse with thin soles, pedal osteitis will respond like that to hoof testers. I can't explain why the films don't match the picture but they are so underexposed it's hard to be sure of anything.

It's not unusual for horse's on restricted diets to develop odd things to do with their mouths. Manure eating is another one, wood chewing. More time turned out with a muzzle may help. Ultra slow

The vet didn’t seem too concerned about the naviculars/heel portion of the foot. He said she responded to the hoof testers around where the edge of the coffin bone is.

You have many options beyond PQ - Legend or Pentosan, oral HA, PEA. The NSAIDs don't interfere with pergolide. What they do is cause gut and kidney damage.
