I've had very good luck with U-Gard for mild ulcer symptoms, it's probably similar to human antacids. If the antacid helps then it probably is an ulcer. Depending how bad the ulcer is it

Triple Crown Safe Starch Forage is okay as part of the feed: ESC+starch is 6.1%, which is great. It is higher in fat, at 6%, than is ideal.

Vet doesn't think she's a good candidate, unfortunately. She also gets
very stressed with joint injections and it takes 3x the normal dose of
sedation to get her quiet - so I have not wanted to put

Is your horse exercising? There are safe ways to add more energy to his diet so he won't be losing so much weight if related to the exercise.
