I just saw this after my post.  So it is okay to feed Timothy Balance cubes soaked and not drained to supplement ALL hay for a few days until I can get to a vet?

I tried to find the answer in the files and I read to NOT drain after soaking.  I am having to feed this until I can get to a vet because Midnight can't eat her hay.  Anything else I should know... it's the long weekend and I'll have to feed her this until Tuesday.

It will give him both more calories and more HC which you could buffer if it was half and half so he had to eat it that way, rather than being able to pick out the unsoaked only which could give him a spike.

How many gallons of water are in the tub? Maybe we can accomplish the same thing by adjusting the soak time.

A horse can have normal ACTH and still have PPID. You cannot monitor those horses with only ACTH. The only published study to date shows the stimulation test usually improves and can normalize but we have no details on signs/symptoms and pergolide doses to go with those results.

Since his insulin and laminitis have been steadily worsening, I doubt you will have control on 0.5 mg.

Hi LJ,

As always, it depends on the circumstances.  If a horse was put on Prascend without a positive test result you may want to take them off the medication prior to testing to see if it's actually needed or not.  This is true whether the test is just a baseline ACTH or a TRH Stim test.

Hi Tonya,

We don't recommend any of the Triple Crown hay products other than the Timothy Balance Cubes.  As per Dr. Kellon's message yesterday: No, their bagged forage is not OK. 

This includes the Safe Starch product.

Soaking is for mini horses as well to prevent choking.

He is on  GUT twice a day and I am feeding 6 ounces psyllium at lunch and night check so psyllium doesn’t interfere with Prascend or his other Uckele mineral and supplements. He has been on Assure Guard Gold for 8 months which has helped but not normal so I am trying another route  

I did call triple crown yesterday and spoke to Stacey their nutrition customer service. She did tell me that their bag grass forage is the same hay as in the Safe Starch.

Hi Dr. Kellon,

I am feeding from hay nets....mostly small hole, slow feed nets.  Four feedings a day, 6am, 1pm, 5pm and 9pm.
May I ask why the half soaked plus 1/2 unsoaked?  Does that give him more calories?

I'm trying to figure how I would achieve that, logistically. 

What about feeding two of the meals unsoaked entirely and two soaked?  Or would you be concerned that the tow unsoaked would spike his insulin?

I guess I'm not clear on what the goal is.....and am having difficulty figuring out how to make the change to his feeding routine (my prep for it)  I currently fill all of the hay nets at once, with specific weighed amounts, then load them in a wheel barrow and bring them around the side of the barn to a large galzinized tub set up on an angle with a bottom drain and hose.  Load the hay nets in and fill with water, set timer for an hour and then drain.  Once drained, I hang the nets on hooks above the tub so that they are ready for the next 24hrs of meals.

Hi Kimberly,

As his basal ACTH on the TRH stim test was already high you do not need to repeat the stim test to see if the dose of Prascend is working at this point.  If the .5mg is not enough to get his ACTH down to the upper teens/low 20s you will want to increase that dose.

Need for TRH Response Test

Mouse was diagnosed last month with PPID by a TRH Response Test after taking him off of Prascend for testing purposes.  We re-started with 1/2 tab after those results and he has been on that dose for over three weeks.  We retested yesterday and are awaiting results.  The Vet did another TRH response test ( as we already now he is PPID) which I did not think was necessary as my belief is that the goal should be to see if the dose of Prascend he is currently on is working.  I thought he just needed ACTH tested.  Am I correct in this thinking?  If we need to retest again, I do not want to do unnecessary testing due to cost.

Maybe this will help, I have been helping an older mare at our barn who has had chronic diarrhea and FWS for years.

We started with eliminating any very stemmy hay. Reduced alf by 50% and added 50% of diet in TC timothy balance cubes split into two feedings, 2oz scoop of Uckele Psyllium in each feeding of cubes, with 1 scoop of Uckele GUT in each feeding of cubes and 1 scoop of Uckele absorb all one time per day in the evening.  I make a mashy of the cubes. I just started adding uckele cocoomega  now that she has been stable for a month.

This mare has had consistently normal bowel movements now for about 4 weeks, when previously she had years of issues.  Her coat is now super shiny and she has so much more energy and she is 28 yrs young.  Even her attending vet was impressed.

Dilly was on 2 Prascend starting 4/22/23, changed to 2 mg chewy.com compounded pergolide on 5/22/23. She was retested 5/22 and her results compared to previous test as follows:

3/06/23      ACTH  41.0     Insulin  15.6     Leptin   4.92
5/22/23                  37.6                 15.93                4.86

What should I increase the pergolide to get a more acceptable result?  Before I ask the vet for a new script, I'm thinking of starting immediately with a 2mg compounded capsule + 1 Prascend.  Then retesting after 3 weeks.

I will update these new results in the case history over the weekend.

I must say I am disappointed the ACTH didn't have a bigger decrease.
