More good news today. Vinnie's triglycerides have come back down to 1100 from yesterday's high of 1500.  He is active and sound and eating well. Our goal is 500 before they stop the IV therapy. 

Dr Kellon, I was reading that hypertriglyceridemia can cause laminitis. Is it possible this was a factor in Vinnie's latest episode?

Also, I'll mostly be mixing it in with the current hay, but am prioritizing the old stuff, of course, so I have room in the hay shed when first cut is ready - the "filler" hay is stored elsewhere for the same reason.

About what I thought but was hoping maybe :)

As for grass, ours is native dryland grasses, not an actual pasture, and not irrigated. The % of edible stuff is below 50% of the acreage during the greenest times. Because of his PSSM2 he needs as much movement as possible, beyond riding, and he just doesn't move around as much in the dry lot. While the grass is actually green (maybe 3 months around here), he's only out for 1/2 hr or so. Once it dies off, maybe an hour, but he comes in on his own because while it's tasty green, it apparently sucks when dead... right now he'd rather graze on old poop than the grass (sigh) - that'll stop once the grass starts growing. They only go out in the early AM while I clean the drylot, and don't go out at all when it's wet

I'm quite careful about it, balancing his PSSM2 needs vs his IR/PPID needs can be... challenging.

He's already had one major PSSM2 episode this year due to not being turned out or more than hand-walked for almost three weeks, plus being stalled at night, due to the horrible weather we had most of February: snow, wind, and temps down to -28f before the wind.

Sounds like you are doing OK to continue Invokana current dose until testing.

Since the manure of your other horse looks the same, that anal residue may be just a small amount of remaining free fluid.

You're doing fine and August retest is reasonable unless there's a change. Remember too that getting PPID/ACTH under control influences insulin.

Yes, I would add in oats - 50:50 mix of "racehorse oats" and Ontario Dehy or beet pulp, working up to 1 pound each, within the first hour after stopping work.

If you know a diabetic who has a glucometer, checking glucose about 15 minutes after stopping work would be enlightening.

Thanks Dr. Kellon.  Her hay is ESC 5.3 and Starch is 1.82 for a total of 7.12. She gets a scoop of ON Dehy Balance cubes with hers supps and that’s it.  

I have been exercising her since she has been sound so walks started in the fall and then exercise increases.   By December we were doing an hour and had added in more trot  (no riding) and were doing 3-4 days a week.  Since January I have been doing 4-5 days a week and increasing the trot duration more and have added in some canter.  During the cold spell we had at the end of Jan and into Feb exercise was sometimes not possible due to temps.  Since we are now in warmer weather we have started back onto our exercise again and up to 4 days a week and increasing the trot and canter durations.  I plan to start riding her more too .  Should I be adding oats as an after exercise meal?

Crackers' Update on Cornell results with continued use of WIRX

Crackers was tested again in February 2021 - last test October 2020.  Here are the results and my observations:

Crackers’ newest test results: 


Glucose 79 mg/dL (previously brought up that this is low but reviewing all past glucose results since 2011 reveals this to be normal for him more than half of the time.  He has never had a level above 90 except when on Metformin which had no effect on controlling his insulin - topping out at over 600 uIU/mL)


ACTH 16.7 pg/mL (really pleased with this! 5 mg compounded pergolide seems to be the sweet spot right now)


Insulin 49.42 uIU/mL (up a bit from last test results of 27.72 uIU/mL but as I have read on this site for the past couple of months, insulin tested during cold weather is unreliable.  It was 40 degrees with a cold wind on top of that on the day of testing and that was the high temp.  Temps had been running into the mid to high 20’s at night and no higher than 40 during the day.  He was tested about 5 hours after his first feeding of soaked hay that day and 6 hours after his feeding of beet pulp with minerals and administering pergolide.)


I am happy with these results and definitely believe that WIRX and his ECIR diet is helping keep his insulin within a safe range.  He is holding below the 80 to 100 uIU/mL. At 80 to 100 uIU/mL he would be at high laminitis danger according to recent postings read here on ECIR Main. 


He is happy, eating and moving well. He kicks up his heels every morning.  I take that as a good sign.


I think I’ve covered the confounding factors brought up regarding previous updates. 

Thank you and I plan on testing again in August unless it is advisable to do so sooner.

I buy "shake" aka "trim" online from hemp farms in Oregon.  I've used several different farms.  **I entered a link to the farm I've used most further below in this email. 
 
You do have to "decarb" (activate) the CBD by baking it.  I bake mine in a Pyrex 9x11 glass cake pan @ 240 degrees for about 30 minutes.  I cover it with a tent (for lack of a better word) of foil because there are terpenes in the CBD and they will escape if you don't cover it, but you need to cover it loosely.  You want the terpenes in there, because they assist the CBD in binding to the CBD receptors.  After it's baked/activated, I keep it in a gallon zip lock storage bag.  It will smell your kitchen up, just fyi, but I run my vent fan and the smell dissipates pretty quickly.
 
Whatever CBD you order will come with test results, that show the amount of cbd per mg.  For instance, the most recent purchase I made is "240mg/g" which means there are 240 milligrams of CBD per gram of shake.  I have a scale I got from amazon that was I think $11 and it works great for this.  My horse has some really bad arthritis (hocks, tmj), especially in his right front foot where he broke his P2 when he was 6 yrs old.  I am dosing him at 3.5 grams 2 times per day.  So he gets a total of 840 grams of CBD 2 times per day.  It's working beautifully.
 
I started out at 200 grams 2 times per day and adjusted upward until I could see that he is really comfortable.  I started at this dose because I had already been giving him a commercial horse cbd product and knew what he had been dosed at with that.  
 
This is the strain currently available with the highest CBD per mg that I could find:  ** https://fernvalleyfarms.com/product/sour-lifter-cbd-shake-trim/   It's testing @ 157.99 mg/g and is called "lifter".  It's what I usually buy and I get a pound at a time.  The amount of CBD varies.  Also, all CBD has a trace amount of THC in it, just so you know.  It is below the legal limit federally, so you can't get into any trouble for having it.  CBD is totally legal and it all has trace amounts of THC in it.  Not even close to enough to cause your horse (or you, if you used it for yourself) to get "high".


Willie has been on CBD for almost 2 years now, with no noticeable side effects.  It's very apparent how well it's helping the 2 times that I have run out.  Within 24 hours his tmj is very apparent.

Dr. K:   I am sorry about my file, I will see what I can do about it. Probably not much techo wizard I am not.

Hay:  It was a terrible year for us, growing conditions were ideal until they weren't.  Too much rain.  This hay is my second cut brome/orchard. He eats about 26 to 30 lbs a day, in a net.  At least this is what I was doing before I went to free choice at nite.  My bales are 450 lbs and two horses on it at nite will finish in 15 to 16 days.  I pull them off in the am.  Then I wait about an hour and give soaked soy hull pellets 1.5c. with a probiotic, salt, aloe vera juice and 12 grams of glutamine.   Around 11am he gets 8 lbs of hay divided into 4 piles on my track.  At 4pm he gets 1cup dry weight rsr beet pulp with 1/2 cup timothy pellets, custom mineral mix from Mad Barn, j-herb 2 tsp., 4oz ground flax, 6 vit.e caps 40grams of spirulina and just recently added chondroitin to get ahead of allergy season.  I give him his meds at that time which includes 1 tab of prascend.  Once done he goes back on the bale.

His manure turns very dark after exposure to air, my other gelding is the same minus the anal residue.

After the bloodwork in the fall my vet thought we should cut back his dose from 300 to 3/4 of that.  So I currently cut the tab at the last zero, if that makes any sense.

Be sure to add triglycerides. What is the S/S of your hay? What else are you currently feeding her?  Has her weight been stable? How much exercise is she getting and when did that start?

I see in Dr. Kellon’s recent post that all horses on Invokana should have Triglycerides testing added into their protocols so I will ask my vet about adding this in to our testing protocols.

If this is incorrect can someone let me know and are there any special procedures needed for this testing as in feeding, pulling supplements and so on.
