I have been looking for the Vit.E product.  
I have found one, Canadian product ingredient list is

DL Alpha Tocopherol Acetate
Flavouring Agent Reg# 981203
DF&C Red#3
recommended dose is 20gr daily (equals 10,000IU)

If this is an acceptable content, I am feeding only 4 gr = 2000 IU, daily.

It sounds like he is not likely to have a metabolic condition, so I think the focus needs to be on trim, balanced minerals for good quality hoof growth, and keeping him comfortable with boots, pads, warmth, etc.

I don't know if there is anything else that needs to be done, specific to a case like this.

You are correct that the ESC and starch are important information to have. The chopped oat straw can also be problematic, so unless you have an analysis for that it would be wise to stop feeding it.

Regardless of the reason for it, you need to reestablish the medio-lateral balance as much as possible. From the amount of sidebone and it's disparity medial-vs. lateral, esp. on the RF, there has likely been an imbalance for more time than just during this laminitic event.

You're doing great in the learning department and Elvie is lucky to be your girl.

Thank you Martha for your reply! I just thought I remembered years ago Dr. Kellon saying the dose could be dropped down in the winter, I could be wrong. My thinking was to see if he could go to 4.0 mg from current 4.5 so that in 2021 seasonal rise he won't be at 5.

Hello Lavinia,

I have added the clean xrays with no test and new pics from yesterday and am interested in getting some feedback to prepare for my next trim. Her frog has partially shed off on both so that’s why it looks the way it does.  Please let me know your thoughts and I should be doing anything different based on xrays.  If needed I can get markups done if you think this is necessary.  Thanks and let me know.

I find it very difficult to know what to feed and why. I read all the post and just get more confused.

It doesn’t seen like there are any answers. Honestly it seems to me the best medicine for EMS/IR is exercise.

I asked about amino acid because I feed Equinety daily. I stopped feeding it for 9 months and Thor lost a great amount of muscle. I started feeding it again and he is looking much better. I don’t see any difference in his comfort level either way.

I tried to find my Case History folder and Photo album in the Case Histories sub-group, but when I search for it, our names no longer show up. I was able to use my signature line and add new photos and updated my case history, but please advise what I'm doing wrong. Grass Mumps, laminitis #photo-notice

Hi - my boarding barn is closed for the holidays and without my awareness, Ero was turned out on pasture Christmas Eve and Day. We had some snow, below zero/low temps and sunny skies these days (terrible conditions I know; I've very frustrated). He came in with "Grass Mumps" (swollen glands - see photos in album) around his jaw on Christmas Day. I've never seen this before, but I have been researching. 

12/25: His temp was 100 in evening, feet weren't noticeably warm. He's stalled at night. Eating/drinking normal. Just swollen glands, but not warm or a problem to touch. 
12/26: Very swollen glands still on both sides. Not lame, but had mild warmth in the front right and an undetectable DP in AM. He was put into a dry lot and given 1g of bute with hay. An hour later his temp was 101.4 (higher, but in normal range. He is blanketed). He seemed okay all day (eating/drinking/walking normal). Rechecked vitals that evening: 99.6 temp, mild warmth in left front and both hinds, slight DP in both fronts. He was given 1g of bute that evening again and was bedded deeper with shavings. His mumps had gone down by evening, but still present. Another boarder, an equine vet home for the holiday, briefly checked him over and said he was stable, but to continue to monitor with 2grams of bute a day for 3 days only (reduce swelling if laminitis - call vet if he gets worse). She suggested Thyro-L supplement for an acute inflammatory episode and until he's at a lower weight. Thoughts? 
12/27: Had 1g of bute in feed this AM. He's walking gingerly, being kept in his stall. I'm going there now to check his vitals and will call the vet. 

The grass mumps were new to me. And I've seen other horses with laminitis, but haven't dealt with it myself yet. Advice or other suggestions welcome. This is horrible.

You need insulin and glucose tested, preferably non-fasting and at least four hours after the first meal of the day.

Just wondering, without testing hay, is lucerne a better option than native grass hay or too much protein. I have a 27 year old retiree positive tested to Cushings & most likely to also have IR. He is on Pergolide & off grass.

how far a part from a meal does l-arginine have to be.  Trying to add it into my horse’s routine but not sure how far away from a meal it needs to be?  Is it like j herb and 20 min before a meal is that ok?

Hi Lavinia,

Thanks for your reply and explaining it all a bit more. That is interesting about the hay analysis as I hadn't realised the difference between dry matter and as fed.I can send off another sample for mineral analysis as I wanted to initially check the ESC as I thought that was crucial.

I will chat to my trimmer about the trim and see if he can do a bit more, and I will try and take some hoof photos too, as that might help with more advice. My vet said that the P3 tends to sink medially anyway, which is why it looks slightly higher laterally.

I didn't realise that boots need to have the breakover correct as well, so I need to investigate that. I am currently using Cavallo boots with thick foam pads with the front cut out to allow P3 relief. Lots for me to learn in a short space of time!

I appreciate the support from everyone on here.

Thank you Dr Kellon for your reply and now I understand a bit more about the OST. I did wonder if the latest result had gone up because I had taken away all the sugar and her body wasn't used to it. 

So the blood test I now need is a resting glucose? Is there anything else that I need as well?

I should have been more clear, Laura.  Pergolide is not a drug you can cut back on when you reach your target so, just as Nancy said, I would not decrease the dose.  PPID is a progressive disease so you should expect to make occasional dose increases.  How much and how often varies by the horse.

L.J. - Thank you!  I stopped Heiro.  Tomorrow is his last Bemer treatment.  We rented the unit for $150/week,  He is getting the red light therapy daily.  That will continue.  He is in soft-rides 24/7.  We only removed them to clean and when we rebandage his coronet band each night.

Dr. Kellon - I have ordered the Phyto-Quench and everything I did not already have for the emergency diet.  Do I take him off Bute 100% as soon as the Phyto-Quench arrives or do I need to slowly step him off from it?
