-- do a search of invokana on this site.  you just started today and want to know when/ if you can lower the dose?  jesse has been on invokana 300 mg since 8/19/19. blood work looks good. urine strip testing ( get the full test strip) looks good and no signs of infection or stop go urination. thought of lowering dose is not something i think about often.

if this is pain not due to an abscess, i would try banamine for a few days. i see no downside to a trial.  full disclosure. not a vet. not even that bright. all the best.

Ok, so I’m still getting mixed messages (from this group) about when it is ok/valuable to test. I realize the best answer is 2+ weeks ago, but that is passed and now we are in the moment. This group states under Diagnosis that it is not good to test for IR during acute Laminitis, and yet I am being told that I can’t do/know anything until I test. He didn’t get into the grain bin, I am almost certain he is IR and his feed is tested but he may be reacting to alfalfa (tested safe levels of sugar/starch). My sense in this moment is that NSAIDs are not of use in this circumstance, correct? I’m sorry I haven’t had a chance to review the proceedings from the 2017 conference, I’ve been checking throughout the day between jobs and talking to vets and labs trying to figure out if I can get an accurate test initiated tomorrow. Even still, it will be days before test results are in and by that time we are out of the 72 hour window when NSAIDs might be beneficial in cases where they might be beneficial. 

Either way, the vet is coming tomorrow. I’d like to know what will be the most valuable use of his time.

Any new studies on Nosodes?

A friend of mine sent me this article on nosodes.  She is considering using them instead of vaccines.  I did a search on this site and most of the info was quite old, and a link to an article that Dr. Kellon provided no longer works. 

I was wondering if there is any new thinking on the use of nosodes instead of vaccinations?  If you had an IR horse that you were reluctant to vaccinate, would nosodes provide at least some protection?

I have not been able to find any studies in my internet search that demonstrates effectiveness of nosodes.

Invokana questions

Smoke has started 300 mg of Invokana today.  We have failed to control anything with all recommendations including 2 weeks on Metformin. 
Can someone help me with a timeline of when to retest metabolic panel after starting Invokana?  I see Urine test needed at about 2 weeks?  Is there any other testing that he will need?
How long does it take to see results, for him to be more comfortable in his feet if this will work and if next blood test shows good results can the dosage be lowered?

I have updated our CH and xrays are up to date.

Added to the former questions about vit C and lethargy not improving I would like to say that the runny eyes are still being a problem and getting a bit worse.

I know this is related to uncontroled PPID but in his case he has never ever had runny eyes, at least since the almost 8 years I know him and it all started almost inmediately after Prascend, since then his eyes has always been watering and now that he is in a higher dose it is getting worse with thicker secretions. I don't relate it to uncontrolled ACTH as he never had it before and his ACTH is controlled or almost controled. I guess he must be now even lower than 31 pg/ml.

Sorry I should clarify when she was on estradiol she was better in the am then got worse as the day went on.


Now that she is on pergolide (1/2 mg day) she is worse in the am and gets better as the day goes on.  If she is suffering from prolactin build up this makes sense maybe since the reduction effects of prolactin wears off after 6 hours.  Does that make sense?

Cody's TRH numbers, titrating Prascend

Hi All,

Cody's Trh numbers cam back at 527 pg/ml (reference interval 9-110).  I will get his case history updated today.  We started him on Prascend a week ago, giving 1/2 tab every other day with his third dose being yesterday.  He is also on 

Ideal Animal Health Kaolin Pectin Suspension for approximately 6 months of ongoing diarrhea which had gotten worse about 10 days ago.  We have tried everything, Ration Plus, Yea Sacc, Probios, ABCs Plus, his teeth were floated, plus a few other things tried.  He has been on the Kaolin pectin for almost 3 days and his manure is looking better.  We make sure several hours pass between his Prascend and his "pink stuff", anyway today he doesn't want to eat and is very listless and lethargic so I need to get the APF (will order it this afternoon) and will not give him his 1/2 Prascend tomorrow.  Did we start his doses too quickly?  How many days should he be on the APF before beginning the Prascend again?  I also started him back on Tri-Amino yesterday.  (He was eating fine until today.)  It scares me that he won't eat, he'll be okay, right?  Should we continue to give him the Kaolin Pectin?

Vet is involved, just won’t see pony til tomorrow. 

I have read (here) several times to not test insulin during acute phase as it may be elevated due to acute pain/stress/inflammation. My vet has already said bute or banamine, so I know I will have to be very specific about what tests I want done tomorrow, essentially acting director. 

Lyme, endogenous ACTH, and insulin? And rads of course. 

Also, on ACTH, I know you recommend Guelph, but do you know that the U of S is not adequate? If it is unknown, what do I have to ask to determine if the are equipped to do the test appropriately?

I’m afraid the literature here vs info in emails is confusing.

My understanding is that IR is metabolic, PPID is its own thing which can exist with or without IR (and vice versa) and those are both hormonal, and that carb overload (as in got into the grain bin) is a different animal. In your email response I feel like you are using the terms metabolic/feed induced to describe carb overload in your first paragraph?

IR/PPID induced versus feed induced laminitis are treated differently from a medical perspective - including how effective or indicated NSAIDs would be. That said, the emergency diet can be instituted without harm in either scenario.  Recommendations for acute care can be found in the 2017 NO Laminitis proceedings (as can a detailed description of how metabolic laminitis is different).

Forget Turmeric.  The foundation for pain control and recovery is DIAGNOSIS. If this is laminitis, Lyme also needs to be ruled out. The acute phase won't just go away without correct diagnosis and treatment.

Your role is to get the trim correct and you can't do that without radiographs.

The vet needs to be involved NOW.

Hi Anna,

With regard to the bute since Wixie has been on it long term you will need to taper her off of it slowly or she will experience a rebound effect.  We usually do that by increasing the period between doses over several days and/or cutting down the amount given at a time.  I know from the CH that you've been giving her 1gram a day - is that in one dose or 2?

Hi Jeanne,

If the trims have lowered the heels on the RF as was needed, then the rotation has been eased considerably - a good thing.

Nerve blocks will only allow her to stand comfortably for having the radiographs done. They aren't going to worsen the situation. Just confine her to deep bedding and/or have her in padded boots after until it wears off so she doesn't move around excessively.

As always, I bow to Dr. Kellon's take on this if it differs.

Hi Cindy,

Yes, there is a fair amount of "tilt" in that view so the view is thru the wings of the coffin bone, which is distorting things quite a bit. Everything below the blue line running horizontally along the lower part of the rad is actually part of the sole and medial wall. It makes it difficult to determine relative sole depth and wall length accurately.
