I used a scale to figure out a 20g scoop, and I give him the 20g scoop with aloe and alfalfa pellets when I first get to the barn, and another 20g with the rest of his supplements, which he hasn’t been eating very well, so for about a week now I give the second dose separately too. The two doses are only a few hours apart. I’m taking him to UCDavis for a chronic respiratory work up on Tuesday. This is our 3rd respiratory episode this year. The first was infectious and passed quickly, the last two seem to be more like allergies that stick around for months. This time, It didn’t seem like he was recovering, then suddenly we had a cold spell 3 days ago and he was not so out of breath from just walking around. I’m glad he’s getting better but afraid his symptoms won’t be bad enough by next week to troubleshoot when I take him to Davis.

Give him 22 grams, by weight, twice a day of Spirulina.

Everything you learn from the ECIR outreach groups is the result of many hardworking people from all backgrounds sharing accurate information for metabolic equines. The ECIR Group is made up of veterinarians, hoof professionals, nutritionists, medical doctors and owners.
 
ECIR Group is over 11,000 international members on Groups.io outreach alone.  By collecting histories and laboratory data from thousands of horses with PPID and EMS, ECIR has been able to carefully monitor responses to treatment and management changes. This is real life, in-the-barn, research and the largest field trial in the world. 
 
There is much more to come. This work can not be done without the help of a growing group of volunteers, many of whom you have never seen.

Definitely a huge improvement.

I'd now be looking to maintain what you have but add a bevel to the bottom of the toes to help with breakover even more. Also need to add more height to the back half of the hoof capsules relative to the front half due to those underrun heels. Ramping the very backs of the heels while leaving the heel buttresses where they currently are will help to maintain the vertical height while assisting those heel tubules to stand up straighter and taller. Definitely agree that those hind toes need to come back while leaving the heels alone.

Do we have to stop ventipulmin (inhalator) treatment or can they be given together?
how many spirulina need to be given to a 1200 lbs horse (who I suspect to be EMS and IR too)

I think I have my signature with my case history figured out. Thank you everyone for your help! I would have given up if not for all the wonderful help from this group.

I will update my case history with more detail as I have time and will definitely update it when I get Ana's test results. 

Regarding Vitamin E, she currently gets 2000 IU in oil capsules.

Jessica - my guy is having asthma trouble too. He’s been on spirulina for a few weeks now with only mild improvement and I’m considering adding joagulan. I hope you’ll post updates, and I will too.

All dopaminergic drugs also suppress prolactin. PPID horses seem to vary in prolactin levels from clear signs of excess like lactation (levels confirmed at Cornell in the past) to no signs at all.

Prolactin is involved in shedding, but not temperature regulation.

PPID runs the entire gamut from no sweating to excessive sweating. You can't use that to guesstimate pergolide dose. Check ACTH.

Spirulina should be the base for COPD but you can add J. Yes, it works for people but again start with Spirulina.

There are quite a few threads re: sweating currently. Is it possible that some of these horses could be well on the back side of the seasonal rise and their seasonal rise dose (for some increased dose) of pergolide is higher than necessary? 

In July, when I reduced pergolide dose (for other reasons) Tula immediately began shedding what was left of the coat she had not shed for summary and came into season. Suppressed prolactin was suspected in her case. It seems that prolactin may have some association with hair growth and possibly temperature regulation?

Interesting thought, Amy. It's hard to say whether it could help because how it helps isn't clear, even with menopause. Studies are mixed, some showing a very  mild benefit and others none at all, even at high doses https://www.ncbi.nlm.nih.gov/pubmed/9469333 .

In any case, the dose of E we suggest for all horses is equivalent to the usual dose used in people for hot flashes.

Jia is lifelong normally

it’s dosed by effect! You note the gums colour and you slowly raise the dose until the gums are a bit pinker start with 1 tea spoon

it is really better to give 2 doses a day morning and evening

My late Arab's first (that I noticed anyway) PPID symptoms were excessive sweating and very gradual loss of muscle tone (not ever to the point of looking bony, but he looked less conditioned to me) on his hindquarters.  He didn't become a wooly mammoth until a couple years later.

Talking about the sweating has me wondering, though, if Vitamin E might help (in the interim before pergolide gets up to dose or if the horse is very early/pre-PPID).  It's prescribed sometimes for night sweats for menopausal women (I know, different hormones) and particularly men on Lupron for prostate cancer.  I'm not sure why the estrogen loss causes sweating, but I wonder if it might be a somewhat similar mechanism to the sweating caused by PPID (and, if so, if Vitamin E would help that as well).  Just a very random thought . . .
