Brooke, you'll receive a response from the most knowledgeable ECIR volunteers. Just as an interim perspective, IME you can add #3 to your list:
3) PPID horses can have a higher and longer seasonal rise than "normal" equines. IME, and it's only n=2 since 2012, some equines really ramp up ACTH during the seasonal rise, and their rise lasts longer than the charted dates, well into December. Also IME, 4) The course of each equine's PPID is unique to that equine. It's a tricky devil. Dr Kellon and others here have the best overview of thousands of cases. The one true thing that worked best for my PPID mares is to suspect uncontrolled ACTH first, whatever is going on. Some of us are chasing a progressive, degenerative process 24/7/365. For some of our equines, the goal posts are never set. Yes I know it’s not as much of a donation but for those like me who are struggling to afford my horses meds and supplements every little bit helps. i have considered using Hylarin B as a joint supplement for Pepper but have not because of the maltodextrin. Since you are very familiar with the uckele products I would like your opinion on using this for an IR horse. Where would you get pectin-lecithin combo? I saw that Europeans have Equitop pronutrin, not available here. SBS has "starting gate" pellets - but again, no amounts of pectin/lecithin/sugars clear. Is there a particular vendor you would use? -- |

Emergency protocol for pony who escaped on to grass?

Hi everyone,
Everything has been going very well in my neck of the woods, Red has been lightly started back under saddle and going well, however this morning I found him off his track and down a laneway with grass, we have had very high winds and I don't know if the wind has blown the gate open or what has happened, I am unsure of how long he has been out, he has a slight pulse in one front, I have been ever so careful with his rehab and am gutted that he has escaped on to grass :( is there anything I can do to stop a laminitis attack? Association between ACTH and gastric ulcers or RDC?
Just got ACTH test results back on my best friend's 18 year old gelding diagnosed with PPID in July 2019. At the time of diagnosis, ACTH was 54 uIU/mL (RI: 10-40 uIU/mL). He was started on 1 mg Prascend. A little over 1 week ago, he was tested again, and ACTH is now 150 uIU/mL. I think we are at the tail end of the seasonal rise, so it's not quite clear why with treatment his ACTH is 3-fold higher than at the time of diagnosis.
There are a few clinical pieces of information that I am wondering if they are responsible for the ACTH: 1) He struggled with RDC (due to chronic Equioxx use) for over a year and just recently within the last 5 months has started to manage it. 2) We now think he may have some issues with ulcers that we will treat with GastroGard. Thought it was the veil, but thinking that is not the case. Based on the hypothalamic pituitary axis, it seems logical that the stress of either of these two conditions may result in increased ACTH activity above and beyond the adenoma itself. However, can it account for this large of an increase? Thanks! Thank you, great information. I called Smartpack and the SmartGut Ultra has 15.7 g sugars/ 80 g serving.... That sounds like a lot.... unless the customer rep got it wrong

So this is a good time to point out that in the original study there were only 38 animals and the statistic reported was: While owners reported laminitis in 37% of animals, 76% were laminitic based on study criteria Which means that the quote in the article LJ posted is a bit misleading, even when given completely.
