Hi Catherine, 

I'm sorry you're going through this with your lovely boy. I do find that 1/4 makes a significant difference with my gal. Trust your gut, you know your horse. Perhaps you can stick with 1/2, if he tolerates it, and retest in a couple weeks? See where that puts his ACTH. 

As for salt. Rather than putting in on his hay pellets you could try sprinkling it on slightly dampened hay. This is the only way I had success with salt and copper/Zinc.

How high is insulin. ?Dr kellon wants diet tight, trim tight and acth well controlled before trying metformin and then invokana if nothing else works.   
I say, if insulin is very high. ie over 200, start invokana immed. Assume dr kellon wouldnt agree, she knows best.  A suffering horse might need urgent treatment??  I am a risk taker with my treatments, based on science

Hi LJ,

I am associating the Prascend with wobbling, etc. He gets this way 12 hours after taking more than half a pill. Dr. Kellon suggested the APF to stop the wobbling and not being able to walk. He is able to walk on a lower dose. When I increase the Prascend, he can't walk. The increased dose makes him so much worse. In my opinion, based on what I see, prascend is not helping and in fact is causing my poor horse to suffer even more. I now have to wait 24 hours for it to leave his system for him to start to feel better. I feel just awful for him.

He is active lamintis. I have tried giving salt in his hay pellets, but he will not eat it. And currently I have trouble with him eating anything but hay. 

I think you have mentioned Invokana before...but no one mentioned metformin. What do they do? How do I get these? Is this something I need to ask my local vet for?

Hi Bonnie,

Woodstock had already been on Prescend at half a pill when I increased to a whole pill and the problems started. So I  backed him down to half pill.  Dr. Kellon suggested the APF and increase to 3/4 pill when I got the APF. So that is what I did. He is getting only 1/4 more than before. I don't think I can cut an even smaller dose. The pills are really hard to cut smaller than 1/4...

Hoof Supplement suggestion

I have used a product called Biotin Plus for a number of year for my mare as a "normal/regular" daily hoof supplement. However, since she had an Laminitis in September I wondering if I should switch to a different product. I'm curious what others use. Someone I know recommended using Farriers Formula Double Strength. Can someone please offer some advice and/or share what you use. I have not been successful at downloading my case history. Thank you.

-what is the goal of apf?   Are you associating the prascend with woobling, etc?-
is this active laminitis?
thinking prascend wont fix everything 

i saw a salt block in ch.  are you giving 1 oz iodized table salt?  2,000 iu vit e in soy oil
thinking your insulin >200 can be painful?

is metformin or invokana on the shelf if beeded?

I know that many horses need an even slower increase of pergolide. I started my horse on APF for several days then began the pergolide, 1/4 mg for four days, then 1/2 mg for 4 days, etc, etc. Some may need an even slower increase??

Hi Everyone,

I don't think the APF is working...at least it is not helping Woodstock. I am on day 4 of using and increasing the Prescend and Woodstock is back to not being able to walk and is wobbling and almost fell down. The first night I gave 8 ml APF and gave him his normal half pill. Day two to tonight I gave him 3/4 a pill. I noticed last night he wasn't moving much but tonight he really is unable as he is so unsteady. I gave him banamine and am praying that he will be ok. I am really concerned and sad for him. Tomorrow I will back him back to half pill as he seems to be ok on that dose.

Thank you for answering my question, Dr. Kellon. I can make those adjustments. I appreciate your input and have a couple more questions.
Chance is a 27 yr old 3/4 Arab1/4 Saddlebred. He has been on joint supplement and for arthritic stifles for years , so I am wondering if I should drop  the joint supplement. I found a joint supplement called Equithrive Original Joint  that has no Glucosamine in it.  All it has on ingredient list is the following, for better or worse...
Resverasym 1000mg, Yielding Resveratrol 700mg, Sodium Hyaluronate 100mg, Grape Seed extract 500mg, and Saccheaeromycea cereusiae 12.5 billion CFU
I am also wondering if I should keep him on some sort of probiotic . He has been on Smartpak Senior Ultra for years and that included hind gut ingredients. It also had  ingredients for his hooves. Chance has had good feet all his life barefoot for years even on trail, until this last few months when he has had two abscesses in August and a week ago. I am trying to put weight back on him as well.  He has been retired from work  for almost a year. Would continuing  a hoof supplement help protect his feet?

It is such a huge relief to know I'll be able to afford Khari's meds for some time yet to come, I don't want anything to get in the way of this new vet being willing to work with me on this!
