I’m sure Dr. Kellon will reply. With my horse  his insulin was refractory. That means that I tried everything , diet, metformin, exercise , herbs, etc. but it did not correct it self. Until I used the Invokana

-- I have taken my 27 year old gelding off of the Senior Ultra supplements because of need to eliminate Glucosamine. I want to give him what he needs for gut health, hoof support, skin / coat. and hind end stiffness . Rep at the company is suggesting his regular gut supplement plus  3 different vitamins supplement with Omega 3s and HA...anyone use a  Smartpak for their Cushings horse?

This is probably endocrine. Different treatments if caught right after they eat the acorns - activated charcoal for kidney and gut toxicity but metformin for starch overload. Just to be safe I'd also ice to prevent laminitis since it's possible there is gut starch overload so a bacterial laminitis. (Was there any diarrhea?).

Metformin needs to be twice a day but you could try the extended release form used in people. That's only once a day - or go with Invokana.

I am not familiar with Invokana. I will look that one up as well as the hygain zero.
I was afraid syringing it might be the answer :( I can do it daily but I board and cannot get it done in the am. But, I guess 1x day is better than nothing.
I had not thought about pain making him sweat. That makes me feel even worse.

Acorn induced laminitis - 5 year old mustang

Don't know if appropriate to post here or not. My overweight mustang developed laminitis after ingesting ripe acorns (not oak leaves or caps) She was seeking them out and greedily scarfing them up. Within a week she showed first symptoms. Vet said this was her 5th case of acorn laminitis this year. Question - would this be considered endocrinated type laminitis (she is probably IR) or toxic related, or a combination of both. Would treatment be the same whatever the type? She did not develop any colic or intestinal problems - so not the rare type of acorn poisoning.

Good job Paige!
I looked at your hay analysis and this is not a difficult hay to balance. The major minerals have a good ratio IMO. Yes, you will need to add copper, zinc and manganese but not all that much. Copper and zinc are surprisingly heavy so the volume isn't terribly huge. The tough part is some minerals are bitter tasting so you may need to gradually up the minerals. If you find your horse doesn't want to eat the added minerals then (using your mineral mix) feed it in a smaller amount starting with a pinch per meal if needed and gradually working up to the full amount.

Does you horse live at your home? You missed checking that box, page 2 of the case history, top of page. If you feed and care for your own horse then adjusting the feed is easier.

High iron hay is typically hay grown in high iron soil (red colored soil). Rain hitting the ground will cause a splash back of muddy water onto the grass which causes a build up of iron on the exterior surfaces of the grass. That soil/iron stays on the grass which is cut and baled into hay. You can try rinsing a sample of this hay then drying it and sending the sample to Equi-Analytical for just an iron test. IMO this iron level isn't horribly high, very easy to balance the other trace minerals to this hay.

My horse has been eating hay from the same grower, same fields for 3 years. This year we got a lot of wet weather which delayed the hay harvest. He got half of his largest field cut and baled and delivered. Heavy rains fell for several days so 2 weeks later he delivered more hay from that field. Iron level of the first hay cut is 115 ppm. Iron level from the 2nd delivery is 677 ppm!!!! I am boarding my horse now and luckily the farm owner has set aside the lower iron hay for my horse.

high manganese

I am considering the purchases of a load of grass hay mix but am concerned about the high manganese.  Numbers are as follows:

Iron  76 ppm
Zinc 14 ppm
Copper 4 ppm
Manganese  169 ppm

Sugar and starch are good, 4.6 and 1.5.  But not sure about the manganese.  Usually I have to be fighting high iron.  To feed 22/lbs  this means I would add 1550 zinc and 547 copper. Does that seem correct? Is it toxic?

I have ordered the Herbal Mune Plus and will recheck labs for anemia in about 6 weeks. I am not going to give him iron supplements. 

Would the Herbal Mune Plus also help with the under belly and sheath swelling/edema? He's had ultrasounds and exams, as well as a sheath cleaning, but the swelling cause has yet to be determined.

I think the best advice for now is to monitor BCS even if you have to clip a patch of coat over the ribs to do so. We're better off underestimating than overestimating with an EMS horse. Much easier to catch up.

Has his coat changed? Hoof pain could be making him sweat. Any films since June?

If he won't eat his meds, syringe them in. Retest insulin after 3 to 7 days on full dose.

I tried forma hoof on my post-laminitic/severe foundered horse. After trying everything, that was another option. I have included photos in his album, and I would NOT recommend it for a recovering horse. It is very hard with no *give*, and these horses need a boot with a very soft cushiony pad.
