Compound pergolide capsules in transport

The pharmacy shipped my compounded pergolide capsules by ground transport and it took 5 days to reach me from N.Y. to Fla. Could they have been compromised because of this? Not knowing how hot the trucks get? Thank you

If this were my horse I would have the vet check her for RAO/COPD as well as checking her ACTH level. As we're in the midst of the rise she may need more pergolide at this time of year.

Steaming does not reduce ESC like soaking does. Lepto and Lyme are both blood tests and if you want to rule them out you should have those tests run as well.

Yes, Costco is the way to go, for sure. I get 500, 1000mg tablets for about $22. Got some at Rite Aid once for $120!!! Yikes!

Heavy breathing

Midnight has been experiencing very heavy breathing after minimal excursion. It has been hotter this year/more days in a row than even last year. But yesterday I took her out when it was still in the 70's and it was only low 80's when we finished our ride that lasted about an hour (around the neighborhood) with a combination of walk, trot and gait. After, she was breathing the way she did when we did major rides only then, she would recover pretty quickly. Yesterday she never recovered until evening when it cooled down. I hosed her twice with cold water and she drank a lot. I'm sure the heat is a big factor but is her PPID/IR coming into play? This did not happen last year or even a few months ago. I would be concerned about airway obstruction except she is fine when it is cool. She is eating, drinking, peeing and pooping just like normal. And her energy when we were riding was great!
We have a week of 90's and 100's coming with temps only going into the mid-low 70's at night and I am going to hate seeing this for all that time. Fortunately, we have no significant humidity. One other thing that I forgot to put in my original post. She also was diagnosed as having EPM in the fall 2021. Since doing the treatments for EPM last fall, she has not exhibited any of the symptoms she had before (she was weak in the back end).

Thank you so much for the information!! I will be asking my vet about it tomorrow (possibly this afternoon if I can catch him doing his rounds at the hospital). You are so correct! Just had a lightbulb moment when I realized that recurrent ulcers were most likely trying to warn us a year earlier that she had PPID. She is getting the antifungal and antibiotic treatment 4 times a day currently. Do you think that is adequate? I will ask my vet about adding antiviral like topical ganciclovir. So would it take care of the possibility of Leptospira, Borellia (Lyme) and also Herpes? I am also going to ask him to rerun bloodwork for ACTH & IR. Anything else I should have him check? Last, what do you think about steaming hay? I have looked at getting one of the steamers from Haygain. Would love your thoughts if that is beneficial? I so appreciate you taking the time to reply to my original post! Very excited to hopefully have found a solution to her rubbing the eye and/or getting new injuries and ulcers. Now just praying I can get this left eye clear of the stomal abcess. Hello Maxine! Angel is on 1 mg tablet a day of pergolide. I will ask the vet to run another blood test on Monday to test her ACTH level and get him to look up what it was in March/April when we ran it. I know it was under 50 but I don't remember the exact number. Is there anything else specifically I should be asking my vet to test while running the blood work?
I'm not familiar with ERU. Is that a blood test as well? I'm guessing that makes her more susceptible for the ulcers/abcesses not healing? I am sure my vet would consult with a specialist opthamologist. Is there someone in particular you know and recommend? You have gotten good advice from other members who have gone through this. I want to comment on the medical end. Recurrent ulcers can be an early sign of PPID, as you probably now appreciate. In addition to the antifungal and antibiotic treatments, I would consider adding an antiviral like topical ganciclovir. The uveitis is likely due to organisms already in the horse's body and activated when the PPID damages the immune system. Possibilities include Leptospira, Borellia (Lyme) and also Herpes. The Renovo can't possibly work until the underlying cause is eliminated. You could also consider adding DMSO topically. Finally, it is best to keep ACTH no higher than the low 20s when you have issues like this. -- Eleanor in PA www.drkellon.com |

You haven't done anything wrong, but something can still go wrong. The timing of this is typical for seasonal ACTH rise, which may mean his pergolide dose was too low. It's difficult to manage a horse that is only positive by TRH stimulation. However, horses with EMS can also react to the seasonal rise with abscesses or laminitis. Another possibility here is that while your hay is below 10%, it's not low enough. That, or your hay is not homogeneous and your sampling was not truly representative. How are you balancing your diet? I know you are a NRC Plus grad but what is your Fe:Cu ratio? Have you ever tested his iron status? Hi Maxine, My horse would not eat her supplements if I added Devils Claw but then I got a big 60ml syringe and started giving it to her that way and she now reaches for the syringe. I give her a Beet e Bite after and I haven't had any problems. I tried smaller syringes but only the 60ml is big enough to take the 3 tsps of powder without getting it everywhere ... and it's just so messy and sticky if it spills. We tested blood several times a year, at a minimum spring and fall. Often again in early winter. His insulin never went below 50 and was often 100. I started to test glucose in the barn when we realized he was moving to diabetic. I may have already stated this was pre-Invokana. He would have been a perfect candidate.

This would be a once a week or every 2 week thing.
We ride to a green belt and I would let her graze there for 5 to 30 min depending on my schedule .
Nancy said 5 min for every hour of work so that might be where we stay when she can do an hour of work!

The short answer is 'it's a crapshoot'. We don't recommend putting an IR horse out on grass without a muzzle at the very least and you still need to very closely monitor them for increasing crest, fat pockets and foot soreness.
