Hi Louise, 

Soles are too thin so nothing should be removed, even flaking "dead" sole, as it does add some protection the longer it's there.  It looks to me like there is a significant medio-lateral trim imbalance on the LF that has put the bony column out of alignment and could explain the soreness.  I am also not seeing rotation of the bony column, which would usually be corrected by lowering the heels.  In your case the heels are already a bit too low and Nugget has what we call capsular rotation, which is due to the toes being too far forward.  This is something that can be corrected by adjusting the trim.  I hope Lavinia will comment, but in the meantime it would be helpful to see a full round of photos on all 4, as described here (please use our naming scheme to label all your images), since he has been trimmed since the xrays (aka radiographs) were done.

Its not clear to me now whether the results posted in the first message were fasting or non-fasting.  If they were fasting then they are waaaaay too high, as you know now.   If they were non-fasting then they are high but not alarmingly so.

Thank you for the advice...I think I've achieved it... hooray! The xray link has worked. Since these xrays he has had a trim to bring back the toe and improve the breakover and the heels are now also lower. He will have another xray 8 weeks after the first which was on the 20th June. He is due another trim in a fortnight. Should I ask my Farrier never to remove any sole? Last time I said it,  he said it was going to come off anyway and he couldn't get the hoof wall as it needed to be with the sole flaking away. 
I've checked all his food,  it comes below the recommended starch and sugar amounts. He's due his blood test tomorrow at last... the vet has been a little delayed in this. I am planning that we'll get everything right for him so he can return to his driving next year x

Dr. Kellon,

With tapering off the bute over the last few days she is laying flat out for many hours making it difficult to get food and water into her.  She lifts her head a bit and I can get at the last couple of inches of water in a small bucket.  We are heading into a heat wave here with temperatures getting to 110 F so I am worried about hydration.

Would you recommend the Devil's Claw then or Tramadol?  We do not have Phyto-Quench here and noticed that in no longer contains Devil's Claw.  Does all Devil's Claw come in the same concentration?  I see here the recommendation is 1 tsp 1x and can work up to 2 tsp 2x.

Sherry! You were reading Relevante's mind on the salt intake...LOL....and mine on getting some video of us on the trail. I'm trying to put together a short little video in Vimeo that uses still shots to show what he and his feet looked like in 2018 at the time of diagnosis and end it with a short video of us today on the trail with him moving out. I've never done this before but Vimeo actually makes the process pretty straightforward. I'll make sure to get some video this Saturday when we ride.  We are expecting some rain this week but Friday and Saturday are supposed to be dry so we should be good to go. He has a chiropractor appointment today so I'll be anxious to see what she says about how he is doing.

LOL on the extra salt. If Relevante is like my kids he'd much prefer not so much salt.  When I dose mine for rides I mix it with kaolin so it's not so nasty but none of us is a fan.  

Would love it if you could get one of your fellow riders to get some pics or video of you with Relevante out on trail - this is such awesome news after the long haul you've had!

Not saying this is the same issue with Zahr but just to give you an idea of how stoic some horses are, my gelding has a pretty significant tear in his superficial digital flexor tendon in his left front leg.  Other than the day the vet was at the barn to evaluate him for possible joint injections he has been lame on it exactly 0% of the time.  He'd been doing 2nd level dressage and was on a 6 day a week work schedule (2 lessons, 1 trainer ride, 2 days of schooling, 1 day hack) and other than being stiff behind occasionally and always working out of it - which is why he was going to be evaluated for joint injections - he never said he had a front leg issue.  

Of course there's no way to know for sure but it's very possible some of the 'I'm stiff behind' was actually trying to compensate for the front end and we've seen enough of that happen in other horses that I wouldn't discount it in Zahr's case either.

Thanks Bobbie. It's truly been a team effort though, with me being a very small part of that team. Every time we ride we take all of you with us. And every time we come back from a trail ride it's a victory lap for all of us.

You need to read the study Dr. Kellon sent you again. There were 28 subjects in the study. 3 subjects reported taking medications concurrently. 

One of the educational purposes of ECIR is to help lay persons read studies and have some understanding of what they are reading. Members can cite studies themselves and often Dr. Kellon or one of the mods with research backgrounds( yes we have them here) will explain if the study was conducted properly, what might be a problem with it, etc. 
This four day study demonstrated that a certain species of dandelion extract produced an increase in urine output with one dose, a smaller output  with the second, and no increase with the third.

Shoot. I thought that we had made some good improvements, but now that they have been pointed out I can see the problems... and his RF out of the two seems to be the one getting away on us this time. I will talk to my farrier and see if she's open to it, but I think I would like to get some mark ups done before his next trim. We need help with this and I don't want to continue on the track of dragging this on and on for poor Zahr. But he is indeed wearing his boots full time right now to give him extra support.

Soaking his hay for the other feedings will be difficult, but I'll see what I can do. He's still presenting like the soreness is stemming from higher up in his hind end, and he warms out of it with some in-hand work. 

Based on what you said Dr. Kellon, I think it makes the most sense to stay the course with his meds until the vet is out to do bloodwork so as not to throw any numbers off. And I have a good amount of prascend on hand to increase his dose asap as needed based on the results.

Thank you all for your advice, I have some re-figuring out to do. And I will post the updated CH right now (mainly the ACTH values that were needed).

Rechecking bloodwork

I will be rechecking Scout’s ACTH, Insulin, Triglycerides first thing tomorrow morning. Every time I ask my vet if we should check glucose, he says no. With Scout now  on invokana is it important to do? Thanks

Hi Lynn,

Unless you're going at endurance/CTR pace or doing a heavy workout it's an extra ounce of salt for every hour worked, not 4x as much.  For your 1.5 hour trail ride that's 1.5oz of salt.

Based on those x-rays his trim needs to be addressed to bring back his toes and allow him to grow some sole. Until he has more sole he needs to be in boots and I would not be forcing him to do any exercise at this point beyond what he offers on his own.

Thanks for this Lavinia. I will move to the well water and start at the legs as you say ( he is not fond of cold water.)  The use of white vinegar is a great idea. I use that for so many different things. 

Okay....I have a really stupid question that I'm embarrassed to ask but here goes. Based on what I've read, to give him the correct amount of salt following a ride (I'm defining his sweat as heavy) am I multiplying 1.25 ounces by 4?  (I'm also basing it on 1 1/2 to 2-hour ride - we wouldn't go longer) And am I only giving this amount right after exertion? I plan to spray his hay. He hates syringing.
