Are these lab results normal or should I re-test?

Hi Everyone,

I posted about my test results a few days ago but havent had much of a response so thought I would try again. Sorry to be a nag but I would REALLY appreciate some advice!

I have been managing my Mare as though she was IR since xmas due to laminitis last spring. We are just coming into spring again and she has just started going lame again, I'm worried that her hooves are going through the same nasty cycle as this time last year so I had her tested for IR... results came back as:

Insulin - 3 mU/L (<42)
Glucose - 5.3 mmol?L (3.3-6.7)

I was sure she was going to come back positive but these results are negative. Are they within normal range? The insulin is very low, is there a likely reason for this? I took them to the lab myself (chilled) and they were processed on site immediately (spoke to the lab) so I doubt there were any handling problems.

She has been getting a light green pick of grass in her yard and I had assumed that maybe it was this grass shooting up that was causing her to go downhill again.... but with a normal test now I have no idea what's going on and how to manage it!

Should I re-test her? Can I assume she is not IR? If not IR what's causing her hoof issues?

I have to get to bed,but just wanted to say that ,if my calcs are correct,her ratio is 4.47 (4.5 rounded off)which makes her compensated IR. Or just under the 'line'.

Tightening up her diet as much as possible may be part of the answer.Even a tiny bit of grass may be,as you already suspect,just too much for her.

I punched these numbers into the IR calculator and the ratio for me came back as 31.80 where 10 and above is normal.

Did I do this wrong or am I using the incorrect units or something? I will attach the actual blood report to my case history file.... here:

I should have just gone to bed last night instead of speaking up.

I have just now got your numbers,too,although I also managed to have 2 other different results with the calculator.
I replied to you earlier this morning but deleted my message in an attempt to avoid confusing the issue for everyone,as well as you!
Sigh.

Let's stick with your numbers,so yes,not IR,and I'll revisit this when I get back in from the stable.

I'll let the hoof people speak to her feet,but the other thing is Jaini's suggestion about mare's and their cycling.It's possible this is playing a part here.

I hope Dr. Kellon has a chance to comment on this.

If she wasn't fasted, those results are reliable. They look like fasting numbers though and if that's the case you need to repeat without fasting.

There are some IR mares who experience problems with hoof pain starting in early spring. If she is IR, I can send you info on checking for that.

You may need radiographs to get more information on her hoof pain issues. Abscesses always a possibility with the wet conditions. Otherwise, your trim is appropriate but the feet are still considerably dished and underrun. You may want to consider something like the Eponas that Linda is using. Should move the thread to ECHoof for that.
