Metformin is one of the most commonly used drugs to treat diabetes in humans and has been shown to lower insulin in some horses.  You can read more about it in the files: https://ecir.groups.io/g/main/files/4%20Insulin%20Resistance/Metformin

That's a decision for your vet to make but no contraindication to DMSO.

I would make it a point to soak every  untested load, no exceptions.  Yes, restart metformin.  Icing isn't going to help.

Hello. This morning I walked in to see my guy shifting his weight on fronts. All feet are hot, and it is also unseasonably warm in NJ. I just got bloods back earlier this week. Have not updated history yet. Insulin still high. ACTH 24.7, Glucose 100, Insulin 76.55.

I started soaking hay this AM. Not tested every load, but every time tested comes back below 10% esc + starch. Feet/x-rays look good. Last xrays a month ago. Feet trimmed last week.  His diet is always low s/s. On J-herb  1 tbl AM and PM. Will put padded boots back on.

Simce I only have reason to believe it is metabolic induced, I shouldn't ice?

Anything else to do or look for? Start Metformin again?

Thanks,
Mara
