Vinnie Setback Today Sore Feet again

So a quick update we feel this is a mild laminitis. He has pretty good pulses in both fronts and the left hind has a mild pulse, right hind hard to tell because it is always impaired from his lymphadema and scar tissue from past issues.

I am going to start him back on pergolide because we think that is likely the cause even though there have been some recent hay issues with the new hay I bought that I started to introduce right before he got sore.

Thanks Cass, the RF sneaker is nailed on the foot.. He has been in a sneaker on the RF since Aug least year, this has always been the more solid foot. He does have some packing material and the sneaker is hollowed near the tip of the coffin to alleviate any possible pressure points.

We think that it is likely breakover, since he is growing quite a bit of foot (toe). We moved our reset up to tomorrow.  Hopefully getting the toe back will make him feel a ton better.

Yes, he's favoring the RF. I'm sure you've kept your farrier/trimmer informed that he looked good in the sneakers for a while and then they stopped working for him on that RF after a while. Maybe he can tweak what's under the sneakers, the pad or hoof packing, to make Vinnie more comfortable. I don't have a lot of experience with different glue-on options, but my Paint is more comfortable with less flexibility in the glue-on.

Update,  so this.is my puzzle. Vinnie is quite sore on hard ground but not on soft surfaces. It seems partly circulatory and part breakover.  He is at 4 weeks this Thursday and it feels like he has grown alot of toe this cycle..even more than he had been. 

I have video on hard surfaces where he does not look like this about a week after getting used to the sneakers. so this is new for him.

Thanks Dr Kellon. I also need to remember that just because he looks better from the outside there is still much healing to do overall.  He was walking so well, I took risks by letting him have freedoms when i knew he was feeling pretty high:) that were stupid on my part. :(  I won't make that mistake again.

Just a little update, Vinnie is continuing to improve.  He is still sore over the medial left when he turns in his stall, this has always been the last part to improve.  Knock wood he will be back to where he was in a week or so.. I think this time I am going to give him trazadone to prevent anymore adrenaline rush partying :)

I know how you feel Nancy, Willie was finally sound at some point a couple months ago so I turned him out and he went bat doo crazy.  He was so sore the next day, and still hasn't gotten sound like he was again.

When you send in blood, ask the vet to send an EDTA for ACTH and serum for insulin and tell the lab to only do the ACTH if insulin is higher than your last test.

Would it still be beneficial to get a TRH stim just to be sure not ppid causing issues?

He is off prascend now, and has been since 1/8.. The Jan test we didn't test for acth. Dec was 20.2 when on prascend, July was 24 when not on prascend.  We thought he may be borderline. He was having symptoms of the veil still in January and based on the Dec teat and starting invokana to get insulin down we opted to stop prascend.

His insulin came down from dec 198.37 to Jan 40.94 three days after starting invokana.

Would the hard muscles release anything that could create a metabolic response? Or are you thinking the same that he just over did it.  We are still sore but improving a bit each day.

Muscles can get hard when they are doing more work than they normally would secondary to weight shifting but they don't have any toxins in them.

Hi Cass, we didn't do any Shockwave on the front feet.  But honestly I am wondering if in releasing toxins from stiff muscles, if it caused a metabolic response.  I imagine that could happen. 

I think I am just going to err on the side of caution like you note.

There are still some things I am curious about especially glucose output from the urine samples ans the test strips from my other post.

Or the potential of a hidden ppid causing a metabolic response.  But in reality while on prascend his acth was normal but insulin through the roof.. so that leads me away from a hidden ppid. ( thinking out loud...lol)

I think I just need to be less careless about his rehab and really stick to a program where there is no trotting til we have a new hoof capsule..

Hi, Nancy. Another idea. I know little of Shockwave therapy and only experienced its use on myself for connective tissue injuries and pain. Is it prescribed by your vet or by your farrier? For which tissues? I was told to expect heat and inflammation and to ice afterwards. An obvious suggestion is to revise or confine the targeted tissues or to hold off on Shockwave until Vinnie is moving comfortably in front with his new sneakers for several weeks or a month. If he were mine, I’d  want to avoid inflammation in and near his hooves for while and really it quiet it down.
