Anyone can't get insulin back to normal?

Last April's blood work was impressive with a big reduction, but insulin was still a little over 363 (normal under 300). I got excited a bit and got him a grazing muzzle (tough-1 easy breathe) and let him go with his buddies for 15 min once a week (when cloudy). The first few times he could not eat with this at all and got really frustrated, but we got it right with short grass. Did not change anything else (still soaked hay, omneity, timothy pellets and WIRX).
Blood work July 13th, insulin went up to 402 by my fault since I let him graze a bit. No more grazing it is.
From your experience, do any of your horses never get back to normal insulin level? I know he must go within normal range, but just wondering.
At least he is doing well and my new hay tested 6.1 (esc + starch) so no more soaking....: )))) I hope. Will test him again soon as well as ACTH to see if we are still good.

Hi Francine,
What is Magic's current weight? How much is he eating per day? Is the hay and timothy pellets being weighed? Total weight being fed (hay + pellets) either 2% of ideal or 1.5% of current - whichever is greater? If he's off the Purina and Blue Seal can you indicate that in your CH?
If he's down to a normal weight and eating an acceptable amount you may want to consider using Invokana for him.

Yes he is off this feed. Only getting madbarn omneity and timothy pellets. I did update my case a while ago.
His weight is good so on 2% and I weight every meal.
Can we get invokana in Canada? Spoke to my vet and he doesn't know.

sone members in canada but direct from their local pharmacy snd get a better price than the internet pharmacies. maybe 19% better price. many pharmacies carry invokana

Yes, these horses can get to within the lab's "normal" range, although we aim to be as low as possible within that range because the upper limit is too high. Some horses never fully 'normalize' but still stay somewhat sound; others don't seem to do well no matter what we do and are good candidates for medications. But most of us can get their insulin down to a healthy level without drugs.
If diet is carefully controlled and cannot be improved (meaning you are already soaking hay), but insulin is still high then exercise is the key (assuming Magic is sound). I would aim for 3-6 days a week of at least 30 min brisk trotting (plus warm up and cool down of 10 min each, walking) at a minimum, and more as his fitness improves.
