ALCAR is used primarily for  nerve (neuropathic) pain. We have seen some members report insulin improvement.  It does not replace a tested and balanced diet.

The cresty neck is a sign of insulin resistance so to get rid of it you need to control the diet and lower insulin.ALCar does help with insulin resistance in studies in other species but it is not going to be the solution by itself.

My mare had her first normal insulin test (26). She is in regular exercise, a good weight, diet working, but her crest is still there. She's had it most of her life. Is there more I should do/can do?

Hi Shera,

Was she on hay only (non-fasting) when that test was taken?  If so, you may want to look at places to tighten up her diet or increase exercise, but you also need to consider if the crest is hard or is it soft and pliable?  A hard crest is more concerning than one that is soft.

She gets 1 cup of Haystack low/low hay pellets with supp's in the am but otherwise had only had hay. Her test was at 1:00. The crest is hard but seems shorter than it was a few months ago. Puzzling since her insulin was normal and her weight is good. Thanks, Sherry!

Hi Shera,

As Nancy mentioned the other day (Re: karo syrup test vs serum (groups.io)) we'd expect to see a number closer to 10 in a non-IR horse.  26 may be in the reference range but is still higher than normal on just hay.  Keep her in work, keep her diet as tight as possible and keep an eye on her weight.

I promise, it was a challenge for me too for more than a brief time. Hang in there.

To be not at risk for laminitis you need a G/I ratio above 10, RISQI greater than 0.32. MIRG is used when glucose is also high, pointing to a diabetic condition that might skew the other two results.   I did not see an updated test in the CH so can't comment further. Updating that info would help us help you faster and give you a more complete picture.

Conditions at the time of the test are also important 
  • The interpretations of results from this calculator are only valid for animals fed nothing but hay or pasture from the night before testing through the time of testing, with hay available at all times. If the horse runs out of hay overnight, wait to test until at least 4 hours after hay was provided again.

I'm thinking of trying ALCAR to help my DSLD mare for neuropathic pain. What would be the dosage for 900-1000 lbs? ForagePlus says 10 grams for 500 kg horse. Feed with alpha lipoic acid?
