Cayuse's laminitis vastly improved - Dr Kellon nailed it


 

Thanks to Dr Kellon's sharp eye, Cayuse is much better - better every day since I stopped AAKG. Years ago I read the red box warning NOT TO USE AAKG during active laminitis, but I forgot. So I had one foot on the gas and the other on the brakes at the same time, so to speak. Cayuse started improving almost immediately, is walking well in her larger clown shoe EasyBoot Clouds, trying to trot at times (No!), no longer lying down several times a day in her stall. Swelling down on all four legs, weight shifting rare, hooves cool to the touch, pulses barely detectable (normal). We'll get her trim working early next week. Just an FYI, this experience proves to me that boots can boost serious heel growth within a month. 

I don't ignore the positive impact of pergolide dose increases and resumption of Invokana removing triggers of high insulin/high ACTH. I'm sure they were critical, too.

With PhytoQuench with Devil's Claw on board for 5 days, I am restarting a slow, traditional taper off NSAIDS along with PQ. I cannot risk loss of appetite, so I plan to use the gradual schedule in Files

She's eating 1 lb BP and mineral supplements and L carnitine, and I'm gradually increasing the amount of BP.  I hope to have results from triglycerides and liver panel thru IDEXX tomorrow. Will report.

I am so glad to read this Cass.  I tried Beau on aakg three times,  and each time he would get worse. Light finally dawned. 

Thanks for showing us how important pulling apart all the factors is to helping our equines. Thanks for shwong us why this group is different from all the others.

I managed to get a glucometer reading today at the blood draw (***trick with the glucometer is to keep it and and test strips in a warm pocket next to my body until right before use and use a drop of blood straight out of the vein without any cooling or preservatives***).  Glucose remains high at 113. I hope that's from testing during the first morning feed and the stresses of laminitis pain. Her former glucose levels were commonly around 80 when taken 4 hours after morning feed.

I'm having trouble arranging a urine sample to check glucose clearance. Her urination pattern has changed, more frequent urination in smaller amounts, so it's hard to stage conditions for a catch. I walk around with a test strip in my pocket but can't deploy it in time.

Is she drinking more? Does she ever start urinating then stop, or dribble? Those could be signs of a urinary tract infection.

I'd say she's drinking normally. Last night she drank the usual amount when she's on Invokana. I haven't tested for UTI since late April, but the strip was negative then. May 7 urine culture didn't grow bugs.

I uploaded today's IDEXX Liver panel with triglycerides. It all looks pretty good, considering. Triglycerides are elevated at 192 mg/dl. I'm not shocked by the result. I expect triglycerides will drop when she get on the full 2 pounds of BP and she's consistently eating all her hay. She's consistently eating about 18 pounds since she leaves about a pound and half every 24 hours.
