Sorry - I did look at the file on this subject but it was a little over my head. Should an IR and PPID horse be fed Acetyl-L Carnitine if the horse's IR Is controlled? Is it a good preventative or simply a good pain reliever?

Research in other species shows a beneficial effect on insulin, but that hasn't been done in horses. It is useful for neuropathic pain in chronic laminitis.
