Thanks Dr. K!  She doesn’t have much of a winter coat sadly as she lives in blankets most of the time due to her sensitivity to cool/wet weather and in the summer bugs drive her crazy (not allergies just make her run).

She is toasty warm under her blankets and we monitor her closely to try to ensure she doesn’t sweat or is cooler than she wants to be.  Currently she is in boots, socks and leg warmers most nights as well and during the day depending how warm it gets she is in boots without socks and leg warmers.  When we go for walks it is without boots and I don’t see any signs of tenderness.

Will keep her feed the same then and you suggest!  I did list a pic of her in my album so I can see if your interested.  Thanks again!!

Cold alone can raise insulin so as long as her feet are comfortable l'd be inclined to leave things as they are unless you see her obviously blimp up.  The winter coat is easily confused with weight gain.

Good to know Sherry.  She doesn’t know at all her hay most times that I put out so reducing hay may not be the answer anyways. Her insulin is up but she seems happy and is active in her paddock running and playing with or without boots. Boots are on due to cold weather and I her previous issues with winter lamintis, so playing it safe.

Yes, I'd want to get some input from Dr. Kellon although you could reduce the overall amount being free fed (if that makes sense) if you think she's getting too chubby again.  There's so much we don't know about horses on this medication and there are more than a few who are doing well but the insulin numbers are higher than we expect/want to see.

yeah that’s kinda what I was thinking too about reducing her feed intake. Her triglycerides levels are lower which I think are ok. However her insulin is up quite a bit from the last test, which is concerning to me considering she is being exercised about the same.

Her feet are doing well and no indication of soreness.  Yep I can upload some pictures.  I know previously Dr. K had told me I could reduce feed if I had thought she was gain a bit of weight but that was a while back and things may have changed with horses on Invokana since then.

Hi Heather,

I'm just catching up but do you have current pictures you can post? I'd be hesitant to drop her off free choice feed as she's on the Invokana.  How is she acting/feeling?  How are her feet doing?

Hello,

I had got some updated bloods done for Riosa and wanted to report them and get some feedback.
 

She is on 300mg/day of Invokana and gets 2 (2.5mg) pills a day of pergolide mesylate.  She never tests positive for ACTH but shows symptoms so we are treating her for PPID based on those.

She currently is on free choice tested hay which is 6.85 s&s along with custom mineral to balance. She is it being ridden at the moment and we are doing handwalking when possible (weather and other issues).

here are her current blood results:

Glucose 5.0 mmol/L. ref range - 3.7-6.7
Insulin 282 pmol/L - ref range > 300
triglycerides - 0.4 mmol/L no ref range

taken on Oct 13, 2022

previous blood results from July, 2022

Glucose - 5.0 mmol/L
Insulin - 166 pmol/L
Triglycerides - 0.8 mmol/L

she was not being ridden or exercised strenuously at that time as was having some other issues. Did bloods to rule out high insulin levels. 


I am wondering about cutting her feed back to not be free choice since increasing exercise is not always possible.  She does seem to have gained a bit more weight over the fall as well.
